不同时期基层医院与大型外科中心在术后早期炎性肠梗阻诊治的异同分析

摘　　要：目的通过比较不同时期基层医院与大型外科中心就术后早期炎性肠梗阻early postoperative inflammatory ileus(EPII)的诊治情况,总结术后炎性肠梗阻的临床特点和有效的诊断、治疗方法。方法回顾性分析广州中山大学孙逸仙纪念医院肝胆外科(纪念医院)和中山市黄圃人民医院普外科(黄圃医院)近14年来收治的69例术后早期炎性肠梗阻患者的临床病例资料。根据不同时间段分为早期、中期、近期3组进行分析。结果早期组病人在两个医院的治疗手段都主要为手术治疗,并发症较多,疗效较差;近期组病人在两个医院均通过保守治疗中取得满意疗效;然而,中期组病人在黄圃医院中仍然主要为手术治疗,而保守治疗在纪念医院同期开始成为主要治疗手段,并取得满意疗效。结论保守治疗在两个医院都能很好地治愈术后早期炎性肠梗阻,外科新观念的普及与推广对提高疾病的整体救治水平有着重要的意义。Objective To compare the different-periods of diagnosis and treatments of early postoperative inflammatory ileus(EPII) in local hospital and large surgical centre,and to summarize the clinical characteristics and the diagnosis and therapeutic methods of EPII.Methods 69 patients of EPII,which were adopted in the recent 14 years in Sun Yat-Sen memorial Hospital of Sun Yat-Sen University and Huangpu People’s hospital of Zhongshan,were divided into 3 groups,namely early,middle and recent group.The clinical data were analyzed retrospectively.Results Surgical treatments were the main choices for the two hospital in the first period,and satisfactory results were not achieved for the both two hospital.However,conservative treatment was used in most patients during 3rd period in both hospital.But in the 2nd period,the therapeutic effects on Sun Yat-Sen hospital were better than that in Huangpu People’s hospital,due to the earlier cognition of the conception of EPII and the application of consevative treatment.Conclusion Conservative treatment can achieve satisfactory therapeutic efficacy on EPII.To publicize and promote the new surgical conception are very important for the improvement of the general therapeutic level for diseases.
