鞘内注射Ad-hNGFβ对神经痛大鼠的镇痛作用及背根神经节Kv1 mRNA表达的影响

摘　　要：目的:观察鞘内注射重组腺病毒介导人神经生长因子β基因(Ad-hNGFβ)对坐骨神经慢性压迫性损伤(CCI)大鼠痛阈及背根神经节(DRG)Kv1mRNA表达的影响,探讨其镇痛作用及可能机制。方法:SD雄性大鼠99只,随机分为3组:A组(CCI+Ad-hNGFβ组)(33只);B组[CCI+ACSF(人工脑脊液)组](33只);C组(假手术+ACSF组)(33只)。术前、术后3、7、14、28d分别测定各组大鼠热痛、机械痛阈值以及疼痛行为学评分。术后3、7、14、28d每组取4只麻醉后处死,取L4～L6段DRG,通过逆转录-聚合酶链反应(RT-PCR)方法,半定量分析Kv1mRNA表达变化。结果:所有CCI动物从术后第3天起,出现明显疼痛行为学改变和热痛、机械痛阈值的降低,与C组比差异有显著性(P<0.05)。CCI术后立即鞘内注射Ad-hNGFβ,术后第14天起,该组行为学评分明显低于B组(P<0.05);术后第7天起,热痛阈值明显高于B组(P<0.05)。RT-PCR结果显示A组和B组术后3dKv1.2,Kv1.4,Kv1.6mRNAs明显减少,显著低于C组(P<0.05),术后7dKv1.1mRNAs明显减少,显著低于C组(P<0.05),术后Kv1.5mRNAs均无减少(P>0.05),术后14dA组与B组比Kv1.1,Kv1.2,Kv1.4mRNAs明显增高(P<0.05)。结论:CCI术后立即鞘内注射Ad-hNGFβ可缓解大鼠神经痛,降低行为学评分,增加热痛阈值,可增加钾通道Kv1.1,Kv1.2,Kv1.4mRNAs表达,说明CCI大鼠痛觉过敏与背根神经节Kv1.1,Kv1.2,Kv1.4,Kv1.6mRNAs表达的减少有关。Objective To observe the effects of intrathecal recombinant adenovirus mediated human nerve growth factor βgene(Ad-hNGFβ)injection on the pain threshold of rats with chronic constriction nerve injury(CCI)and its influence on the expression of Kv1mRNA in dorsal root ganglion(DRG),and to explore the possible mechanism of its analgesic effect.Methods Ninety-nine male SD rats were randomly divided into three groups:CCI+Ad-hNGFβ group(group A),CCI+ACSF group(group B),control group(group C).The threshold of thermalgia and mechanical pain,pain behavior score were determined at the following time points:preoperation,3rd,7th,14th and 28th days postoperation.The DRG of L4 to L6 segment were taken from 4 rats of every group after sacrificed at 3rd,7th,14th and 28th days postoperation,and the expression of Kv1mRNA was determined with reverse transcription-polymerase chain reaction(RT-PCR)method.Results The obviously change of pain behavior and decrease of thermalgia and mechanical pain were observed at 3rd day postoperation in all CCI rats,and the change and decrease in CCI rats were significant difference with those of control group(P<0.05).The intrathecal Ad-hNGFβ injection were given postoperation in group A and B,the pain behavior score in group A was significantly lower than that of group B from 14th day postoperation(P<0.05),the threshold of thermalgia in group A was significantly higher than that of group B from 7th day postoperation(P<0.05).The results of RT-PCR showed that the expression of Kv1.2,1.4,1.6mRNAs were significantly declined at 3rd day postoperation,the Kv1.1mRNA was declined at 7th day postoperation in group A and B,while significantly lower than those of group C(P<0.05),the expression of Kv1.5mRNAs had no significant difference postoperation.The expression of Kv1.1,1.2,1.4mRNAs in group were significantly higher than those of group B at 14th day(P<0.01).Conclusion Intrathecal Ad-hNGFβ injection after CCI operation could alleviate the nerve pain,degrade the behavior score,increase the thresold of t
