彩色多普勒超声联合盆腔血管造影诊断子宫动静脉瘘

摘　　要：目的:探讨彩色多普勒超声联合盆腔血管造影对子宫动静脉瘘的诊断和治疗作用。方法:筛选2001年1月-2009年12月因行引产、人流术、清宫术及诊刮术后出现阴道不规则流血或突发性大流血来我院就诊的患者,对其行彩色多普勒超声检查。对彩超检查提示有子宫动静脉瘘形成的患者行盆腔血管造影,并对确诊为子宫动静脉瘘的患者行子宫动脉栓塞术。结果:彩超检查提示有子宫动静脉瘘形成的患者45例,其中39例接受盆腔血管造影检查,28例患者确诊为子宫动静脉瘘且在造影下行子宫动脉栓塞术,术后治愈出院。结论:通过彩色超声结合盆腔血管造影检查可明确子宫动静脉瘘诊断,同时行子宫动脉栓塞术是有效的治疗方法。Objective To explore the value of color doppler ultrasonography combined with pelvic angiography in diagnosis and therapy of uterine arterivenous fistula.Methods Patients with irregular vaginal bleeding or abrupt vaginal hemorrhea that bled after induced abortion or curettage were selected from January 2001 to December 2009.The color doppler ultrasonography were undertaken in these patients,which would demonstrate whether need to undertake the pelvic angiography or not.The patients with uterine arterivenous fistula confirmed by pelvic angiography were treated with uterine artery embolization.Results Total 45 patients were demonstrated with formation of uterine arteriovenous fistula by color doppler ultrasonagraphy,39 cases among them were undertaken pelvic angiography.Finally,28 cases were confirmedwith uterine arteriovenous fistula and treated with uterine artery embolization.Conclusion Color doppler ultrasonography combined with pelvic angiography would be valuable for diagnosis of uterine arterivenous fistula,uterine artery enbolisation could be an effective treatment method.
