摘  要:目的:观察脑出血大鼠脑组织中血管内皮生长因子(vascular endothelial growth factor,VEGF)的动态变化,以及"百会"透"曲鬓"针刺法对VEGF的动态调控,探讨该针刺法治疗脑出血的可能作用机制。方法:采用自体血注入法制备脑出血模型;实验大鼠分为空白组、模型组和针刺组。针刺组针刺病灶侧"百会"透"曲鬓"穴。应用神经行为学指标观察各组大鼠神经功能缺损程度;在电镜下观察各组大鼠神经细胞胀亡变化;运用免疫组化方法动态测定不同时间点各组大鼠脑组织中VEGF的表达。结果:①空白组的大鼠表现正常;各造模组术后均出现不同程度的神经功能缺损,以2~3 d时最为严重。针刺组大鼠神经功能缺损征较模型组有明显恢复,与模型组比较,6 h、1 d评分无差异,无统计学意义(P>0.05);而2 d、3 d、7 d评分有差异,有统计学意义(P<0.01)。②电镜下可见模型组大鼠术后6 h可见神经细胞胞浆内线粒体肿胀,粗面内质网扩张;2 d^3 d时神经细胞胞体胀亡最为严重。针刺组较模型组神经细胞胀亡减轻,神经细胞膜及核膜破损程度也较轻,线粒体肿胀呈空泡较模型组相应减少。③模型组和针刺组大鼠术后均出现不同程度的VEGF阳性细胞表达增多现象。模型组大鼠术后6 h即出现大量的VEGF阳性细胞表达并出现第一个高峰;1 d、2 d出现缓慢下降态势;3 d突然增高并达峰值;7 d时阳性表达仍高于空白组。针刺组大鼠在1 d^3 d时间点,VEGF的表达明显低于模型组(P<0.01);而在7 d时间点,针刺组VEGF阳性表达反而较模型组明显增多(P<0.01)。结论:①脑出血大鼠脑组织内VEGF表达上调,且其变化与行为学评分和神经细胞胀亡的形态学变化相关;②头针疗法在脑出血早期能够降低鼠脑内VEGF阳性细胞表达,而在脑出血后期又可促进VEGF阳性细胞表达,从而提示针刺在脑出血急性期可能具有良性的双向调节作用。Objective:To investigate the effects of BaiHui-QuBin scalp-acupuncture on VEGF expression in the rats with intracerebral hemorrhage(ICH) for exploring the interaction mechanisms.Methods:All rats were made to be the intercerebral hemorrhage model.Experimental rats were randomly assigned into the blank set,the model set and the pinprick set.The rats of the pinprick set were acupunctured on the trouble side by BaiHui-QuBin scalp-acupuncture.We observated the damage extent of the nerve function of each set in deferent time phase point with the neuroethology index;observated the change of the brain histomophology of each set in deferent time phase point with electron microscope;detected the positive expression of VEGF in brain tissue of each set with the methods of immunohistochemistry.Results:①Compare with the model set,the pinprick set’s nerve function defection and damage was abatement.Both were relatively showed the significant difference(P<0.01)and havd the statistical significant from 2 day to 7 days.②The patho alteration of brainrissue under electron microscope the pinprick set is more significant than model set.③All of the rats after being made model appearanced the phenomenon of multiplication of expression of VEGF.The rats of the model set appearanced the phenomenon of considerable multiplication of expression of VEGF in 6 hours and to the first peak,from 1 day to 2 days appearanced the descending tendency,and to the peak in 3 days suddenly and still to remain higher than blank set in 7 days.From 1 day to 3days,compared with the model set,the pinprick set’s expression of VEGF was lower obviously and were relatively showed the significant difference and had the statistical significant(P<0.01);however,the pinprick set’s expression of VEGF was higher obviously in 7 days compared with the model set(P<0.01).Conclusion:The expression of VEGF is up-regulated in ICH rats’brain.The therapy of BaiHui-QuBin scalp-acupuncture can reduce the expression of VEGF in the earlier and promote the expression of en
