血清CA_(125)在肺癌诊断价值探讨

摘　　要：目的 探讨血清糖链蛋白CA_(125)作为肺癌肿瘤标志物的价值。方法 肺癌患者、其他恶性肿瘤患者、良性肺病患者及正常对照组各63、48、44、50例。血清CA_(125)检测采用微粒子捕捉酶免疫分析(MEIA)法在美国雅培IMx全自动免疫分析仪上进行。结果 肺癌、其他恶性肿瘤患者、良性肺病及正常对照组血清CA_(125)水平分别为(112.6±169.9)、(29.1±36.9)、(22.1± 26.3)、(18.2± 9.8)U/ml;肺癌组显著高于其他三组(P均<0.01)。以35U/ml为临界值,CA_(125)诊断肺癌的敏感度、特异度、阳性预计值、阴性预计值、准确率分别为66.7％、94％、93.3％、69.1％、78.8％。CA_(125)与肺癌分期密切相关(P<0.01),与组织病理类型无关(P>0.05)。结论 血清CA_(125)是肺癌的良好肿瘤标志物,值得临床应用。Objective:To study the value of CA125 as a tumor marker for lung cancer. Methods:Serum CA125 was measured using the Microparticle Enzyme Immunoassay (MEIA) technology with the IMx system in 63 cases of lung cancer,48 cases of other malignant tumor,44 cases of benign pulmonary disease and 50 cases of normal control group. ResultS:The serum CA125 level of lung cancer (112.6 ± 169.9 U/ml) was significantly higher than that of other malignant tumor (29.1 ± 36.9 U/ml), benign pulmonary disease (22.1 ± 26.3 U/ml) and normal control group (18.2 ± 9.8 U/ml). Taking 35 U/ml as a critical value for the diagnosis of lung can-cer , the sensitivity, specificity, positive predicted value, negative predicted value and predicted accuracy of diag-nosis were 66.7% , 94% ,93.3% , 69.1% and 78.8% respectively. Serum CA125 level was closely associated with the TNM stage, but no relationship was observed between the CA125 level and the histopathology type in lung cancer. Conclusion:Serum CA125 is a good tumor marker for lung caner. It is worth widely using in the clinical work.
