老年人代谢综合征与血清铁蛋白间的相关性研究

摘　　要：[目的]探讨老年人血清铁蛋白与代谢综合征及其不同组分间的关系。[方法]对1645名老年人进行血压、身高、腰围测量,称重。检测血清胆固醇、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇、甘油三酯、空腹血糖与血清铁蛋白。[结果]有代谢综合征者比无代谢综合征者的血清铁蛋白值高(P<0.05)。血清铁蛋白四分位各组间比较空腹血糖、甘油三酯、HDL-C、BMI、腰围、收缩压、舒张压(P<0.05或P<0.01),四组间年龄、总胆固醇、LDL-C无差异。与血清铁蛋白呈正相关联的参数有腰围(P<0.001)、舒张压(P<0.001),空腹血糖(P<0.01)负相关联的参数为HDL-C(P<0.001)。[结论]在老年人中,有代谢综合征者比无代谢综合征者的血清铁蛋白值高,随着血清铁蛋白的递增空腹血糖、甘油三酯、体重指数、腰围、收缩压、舒张压增加,HDL-C下降。以腰围、舒张压、HDL-C、空腹血糖与血清铁蛋白关联较大。[Objective]To investigate the relationship of serum ferritin level between metabolic syndrome and metabolic disorders among the elder.[Methods]A cross-sectional study was performed among the people of health examination who were apparently healthy,1645 elder persons were included in this investigation.The questionnaires,physical examinations,laboratory tests were performed.[Results]Metabolic syndrome persons had significantly higher serum ferritin levels than controls(102.4±65.95 vs 82±59.20 ng/ml,P<0.001).The fast plasma glucose,serum triglycerides,BMI,waist circumference,blood pressure were significantly higher in level of serum ferrit than lower levels and increased with progressively higher serum ferritin levels(P value for trend<0.05).While HDL cholesterol was significantly lower in level of serum ferritin and decreased with progressively higher serum ferritin levels(P value for trend<0.05).Stepwise multiple regression showed that waist circumference,diastolic pressure,fast plasma glucose,HDL cholesterol entered into the original equation.[Conclusions]Serum ferritin level is significantly associated with metabolic syndrome in elder,waist circumference,diastolic pressure,fast plasma glucose,HDL cholesterol are closely associated with the risk for serum ferritin.
