摘 要:目的用弥散张量成像磁共振技术观察首发未用药的重性抑郁障碍患者脑白质的异常。方法对35例重性抑郁障碍患者[女18例,男17例,年龄(28.37±9.70)岁,受教育年限(12.18±2.82)年]和35例性别、年龄及受教育程度相匹配的健康对照者[女18例,男17例,年龄(29.25±11.09)岁,受教育年限(11.81±2.45)年]进行3.0T弥散张量成像磁共振扫描,采用基于体素的分析方法来研究重性抑郁障碍患者全脑白质异常。结果与正常对照相比,重性抑郁障碍患者左侧额叶上纵束(-40,21,24)、左侧额内侧回(-10,2,52)、左侧顶下小叶(-54,-38,21)的脑白质FA值显著降低。结论重性抑郁障碍患者存在上纵束、额叶、顶叶的脑白质异常,这些异常可能是重性抑郁障碍发病的重要病理生理机制。Objective In this study,3.0 T diffusion tensor imaging (DTI) was used to examine white matter abnormalities in single-episode,medication-naive major depressive disorder(MDD) participants.Methods DTI datum were acquired from 35 MDD[18 females and 17 males,age(28.37±9.70)years old,education years(12.18±2.82) years] and 35 healthy control participants[18 females and 17 males,age(29.25±11.09)years old,education years(11.81±2.45) years].A voxel-based analysis(VBA) was used to investigate whole brain white matter abnormalities.Results Fractional anisotropy was significantly decreased in the left superior longitudinal fasciculus (SLF) within the frontal lobe(-40,21,24),left frontal(-10,2,52) and left parietal(-54,-38,21) white matter in the MDD group compared to the healthy group.Conclusions Our results demonstrate abnormalities in SLF,frontal and parietal white matter in the early stage of MDD and suggest that these abnormalities may play a key role in the pathophysiology of MDD.
