高血压并打鼾患者阻塞型睡眠呼吸暂停综合征患病状况和特点分析

摘　　要：目的研究高血压并打鼾患者中阻塞型睡眠呼吸暂停综合征(OSAS)的患病状况及其特点。方法选择2004～2009年在新疆维吾尔自治区人民医院高血压科诊治的1193例高血压并打鼾患者,经多导睡眠呼吸监测仪诊断OSAS。分析OSAS的患病状况,不同性别、年龄、体重指数OSAS的患病特点。结果 1193例患者中83.3%诊断为OSAS,其中中重度OSAS占55.0%。男女比例4.4:1,男性患者中重度者多于女性,女性患者以轻度OSAS为主。患者中41.2%超重,48.8%肥胖,肥胖患者中OSAS占89.2%,同时随着体重增加OSAS呈现加重的趋势。OSAS检出率随年龄增加而逐渐增加,<40岁患者OSAS检出率71%～77%,40～60岁为82%～87%,>60岁患者明显增加至92%。男性OSAS患者以40～50岁居多,女性以50～60岁居多。结论高血压并打鼾患者OSAS检出率高,半数以上合并中重度OSAS。在此患者人群中进行多导睡眠监测,对明确高血压的病因及针对OSAS的诊治具有重要的价值。Objective Obstructive sleep apnea syndrome(OSAS) has been associated with increased cardiovascular diseases.This study is to explore the prevalence and character of OSAS in the patients with hypertension and snoring.Methods 1193 patients with hypertension and snoring were investigated in our hospital from 2004 to 2009 and ware analyzed the prevalence and clinic characteristic of OSAS.Results The incidence rate of OSAS in the patients with hypertension and snoring was 83.3%.The moderate and severe patients with OSAS in men were higher than in women who were mainly the mild patients with OSAS.The prevalence of OSAS had been increasing along with age increasing and was high during 40-50 years old in men or during 50-60 years old in women.Conclusions The incidence rate of OSAS in the patients with hypertension and snoring is high.More than half of the patients were moderate and severe ones.Polysomnography(PSG) should been taken in these patients in order to find one of the causes of hypertension and to promote correlated treatment.
