摘  要:目的探讨血必净对油酸诱导急性肺损伤(acute lung injury,ALI)的可能作用机制。方法健康SPF级雄性Wistar大鼠45只,随机分为三组(n=15):A组注射生理盐水作为正常对照;B、C两组所有大鼠均用油酸复制ALI模型,B组为模型对照组,C组在模型基础上加用血必净为治疗组。三组均于注射后12h抽动脉血作血气分析测定血氧分压,确定模型成功后,A、B两组经尾静脉注射生理盐水,C组经尾静脉给予血必净,给药7d,各组于给药后第1天、7天、14天分别处死5只大鼠,取肺组织、心脏血及左肺行支气管肺泡灌洗,肺组织病理学检查并检测灌洗液及血清Ⅲ前胶原(PCⅢ)含量。结果与A组相比,B、C两组动脉血氧分压(PaO2)明显下降(P<0.05),但A、B两组间差异无统计学意义(P>0.05),与A组相比B、C组各时点血清和灌洗液PCⅢ明显增高(P<0.05),肺组织病理学改变均明显,C组与B组相比各时点血清和灌洗液中PCⅢ浓度降低(P<0.05),组织病理学改变明显减轻。结论血必净可能通过抑制ALI炎症反应和肺纤维化对肺损伤起保护作用。Objective To investigate the protective effect of Xuebijing on oilec acid-induced acute lung injury(ALI).Methods Forty five specified-pathogens free(SPF) Wistar rats were randomly divided into 3 groups of 15 animals each:group A,as normal group.Rats of group A were injected with normal saline(NS).ALI model were injected intravenously with oleic acid in B and C groups.Twelve hours after injection,artery blood were drawn for blood gas analysis in all rats.Then the rats in the C group were treated with caudal vein injection of Xuebijing every day from the 1st to 7th.The rats in A and B groups were treated with NS.Five rats in each group were randomly sacrificed on the 1st,7th,14th day,respectively.Histological examination was performed with HE staining.Venous blood and bronchoalveolar lavage fluid(BALF) were obtained for detection of PCⅢ concentrations.Results Compared with group A,oxygen pressures of artery blood(PaO2) was reduced significantly in group B and group C(P<0.05),but there was no significantly difference between group B and group C(P>0.05).The concentrations of PCⅢ in the plasma and BALF were significantly higher in group B and C than in group A(P<0.05).PCⅢ of the plasma and BALF were decreased significantly after Xuebijing administration(P<0.05).There was massive inflammatory cell infiltration in the alveoli and interstitial tissue and excessive deposition of collagen in the interstitial tissue in group B.The inflammatory and fibrosis were less serious in group C than in group B.Conclusions The result suggest that a treatment with Xuebijing may be useful in inhibiting inflammatory and pulmonary fibrosis in the process of ALI in rats.
