肺癌三维适形放疗中脊髓剂量计算的研究

摘　　要：目的:介绍基于L-Q模型推导出的EQD2=D1[(d1+α/β)÷(2+α/β)]的临床应用,比较肺癌3DCRT中,脊髓的物理剂量与生物剂量,以期达到评价的统一。方法:为7例病理证实的原发性肺癌患者,设计治疗计划,利用TPS的剂量体积直方表计算出每次照射脊髓的最大剂量,将其设为d1,代入公式EQD2=D1[(d1+α/β)÷(2+α/β)]计算,求出d1的生物剂量EQd1;同时求出D1的生物剂量EQD2。将d1与EQd1、D1与EQD2进行比较。结果:d1与EQd1的均数有显著性差异(P<0.05);D1与EQD2的均数有显著性差异(P<0.05)。结论:肺癌3DCRT计划评估时脊髓用生物剂量EQD2,不主张用物理剂量。Objective: To introduce clinical practice of based on that L-Q model which derives out EQD2= D1 [(d1+α / β)÷(2+α / β) ],compare physical dose and biological dose of spinal cord in three dimensional conformal radiotherapy of lung cancer,in the hope of reaching the unity.Methods: Totally 7 patients of primary lung with pathologically confirmation,when design treatment plans,the use dose volume histogram table of TPS to calculate the maximum dose of the spinal cord each time,set it to d1,into the formula EQD2=D1 [(d1 + α / β)÷(2 + α / β)] calculations,to obtaine EQd1 of d1,EQD2 of D1.To compare d1 with EQd1and D1 with EQD2.Results: The d1 and EQd1 are significant differences(P<0.05),D1 and EQD2 are significant differences(P<0.05).Conclusions: When to trad the spinal cord in 3DCRT of lung cancer with EQD2,we do not advocate uses of the physical dose.
