摘  要:目的:研究放创复合伤大鼠伤口愈合的病理变化,以期为进一步阐明放刨复合创伤难愈的机制奠定基础。方法:用光镜、电镜和免疫组化染色等方法观察单创伤和放创复合伤伤口局部炎症反应、组织修复反应及细胞外基质(胶原和纤维连接蛋白)含量变化。结果:与单创伤相比,复合伤组大鼠伤后早期炎症反应显著减弱,表皮再上皮化显著延迟,伤口局部成纤维细胞数量和肉芽组织形成显著减少细胞外基质含量显著减少。结论:放创复合伤大鼠伤口局部炎症反应减弱,成纤维细胞数量显著减少、功能明显受抑,表皮再皮化显著延迟,这可能是导致创伤难愈的重要原因。Objective:Pathological changes in incision wound of skin after whole-body irradiation (WBI) were studied to further elucidate the mechanisms underlying impaired wound healing.Methods:The inflammatory re- sponse and tissue repair reaction in wounds were surveyed by histological methods,and the contents of extracellu- lar matrix (type Ⅰ collagen and fibronectin) were determined by immunohistochemistry.Results:Inflammatory re- sponse was significantly weakened,re-epithelialization was significantly delayed,the number of fibroblasts was significantly decreased,and the contents of extracellular matrix were also significantly decreased in wounds in rats combined with WBI injury as compared with simple incision wound.Conclusions:Inflammatory response at wound site was significantly attenuated in rats when combined with WBI injury as compared with simple incision wound. The amount of fibroblasts were reduced,showing inhibition of their function in the wound site in rats when com- bined with WBI injury.The combined injury affects the process of inflammatory response,wound cell repopulation and tissue remodeling,probably constituting one of the important reasons why the healing of wound with combina- tion of WBI injury is significantly impaired.
