心理疏导在老年人安全拔牙中的作用

摘　　要：目的：进行无心电监测条件下对老年病人施行安全拔牙的临床研究。方法：通过询问病史、检查了解病人全身健康状况,设计合理的拔牙路线及方法,并准备氧气及急救药物；在局麻下行拔牙术,术中随时观察和询问有无不良症状发生,辅以暗示、安慰及音乐等心理治疗手段。结果：59例病人均在无心电监测条件下,通过进行心理疏导和局麻下一次拔除所有需要拔出的患牙,人均每次拔牙8．8颗,时间一般为10～30min,其中仅4例在麻醉后或术中出现短暂头晕、心慌等不适,经对症处理后均缓解,并完成拔牙过程。结论：结果提示,在拔牙前和拔牙过程中进行有效和可靠的心理疏导,可以缓解病人对拔牙的恐惧心理,结合完善的麻醉和拔牙技术,多数老年病人可以完成安全的拔牙手术。Objective:To study the safe tooth extraction in elderly without electrocardiographic monitoring.Methods:The general condition of patients were collected by inquiry and routine examination,the pro- cedure and methods of tooth extraction were planned,oxygen supply and the emergency drugs were prepared. Tooth extraction was performed under local anaesthesia,observation and inquiry for uncomfortable response were carried out carefully,and the psychotherapy was conducted including hint,consolation and music.Results:Tooth extractions were accomplished in 59 elderly patients following the above strategies,and a mean of 8.8 teeth were pulled out in one patient.Conclusion:Effective psychotherapy might reduce the patient's fear and anxiety during extraction of teeth.Therefore extraction of teeth can be accomplished safely in elderly patients with faultless an- aesthesia and extraction technique.
