血小板颗粒膜蛋白与妊娠期高血压疾病凝血状态的关系探讨

摘　　要：目的:探讨血小板活化程度与妊娠期高血压疾病的关系。方法:采用流式细胞仪全血荧光标记法,测定住院分娩的孕产妇233例,其中正常妊娠产妇125例,妊娠期高血压患者108例,检测其产前、产后外周血中活化血小板内α-颗粒膜蛋白(CD62P)及溶酶体完整膜蛋白(CD63)水平,同时采用全自动血凝分析仪测定其产前、产后凝血酶原时间(PT)、活化部分凝血活酶时间(APTT),对以上各组数据进行比较分析。结果:分别比较各组产前PT、APTT、CD62P、CD63值,均具有显著性差异(P<0.05)。产后重度子痫前期PT、APTT较产前延长,而CD62P、CD63较产前降低,差异均有统计学意义(P<0.05)。CD62P、CD63表达率分别与PT、APTT时间呈明显负相关。结论:妊娠期高血压疾病存在血栓前状态,随着病情的加重,CD62P、CD63逐渐升高,PT、APTT逐渐缩短,血栓前状态的程度也明显加重,产后24 h内仍是血栓病的高发时期。Objective:To discuss the relationship between platelet alpha granule membrane protein and Blood Coagulation State of pregnancy hypertensive disorders.Methods:The antepartum and postpartum peripheral blood platelet alpha granule membrane protein(CD62P)and the lysosome complete membrane protein(CD63) in platelet activation were measured by the method of whole blood fluorescent marker of flow cytometric analysis in 233 maternal.Among them,125 Normal maternal,108 pregnancy patients with hypertensive disorders.At the same time,the antepartum and postpartum prothrombin time(PT) and activation part blood coagulation live enzyme time(APTT) were measured by automatic blood clots analyzer.The data of each group were analyzed and compared.Results:The level of PT,APTT,CD62P,CD63 were Respectively different between each antepartum group(P<0.05).The change between antepartum and postpartum : the PT and APTT level were extended in heavy-preeclampsia(P<0.05).The level of CD62P and CD63 were obviously lower in heavy-preeclampsia(P<0.05.Conclusion:There is increased markers of prothrombotic state in pregnancy patients with hypertensive disorders.The amount of CD62P and CD63 are higher and the time of PT and APTT are shorter as the bad condition.The prothrombotic state are significantly aggravating.The time of 24 hours after birth are the period of high incidence of thrombosis.
