摘  要:目的研究Neuregulin-1β对容量超负荷所致心衰大鼠的治疗作用。方法 34只SD大鼠随机分成心衰组、治疗组、假手术组。心衰组和治疗组采用腹主动脉-腔静脉穿刺造瘘法制备模型。假手术组只行主动脉穿刺不造瘘。治疗组造瘘术后8周给予Neuregulin-1β尾静脉注射,10μg/(kg.d),共7d。心衰组和假手术组给予同等剂量的生理盐水。12周后分别采用心动超声、血流动力学、血浆BNP浓度评价心功能;行病理学检查及观察心肌组织的超微结构。结果①心动超声显示治疗组左室射血分数、短轴缩短率高于心衰组(P<0.05),左室收缩末内径、舒张末内经小于心衰组(P<0.05)。②血流动力学检测显示治疗组左室收缩末压和左室内压最大上升、下降速率均明显高于心衰组(P<0.05);左室舒张末压低于心衰组(P<0.05)。③治疗组血BNP水平明显低于心衰组(P<0.01)。④与心衰组比较,治疗组光镜及透射电镜下心肌细胞受损程度明显减轻。结论 Neuregulin-1β可明显改善容量超负荷心衰大鼠的心功能、减轻心肌细胞的损伤、纠正心衰。Objective To study the therapeutic effects of neuregulin-1β on the volume-overloaded rat model with heart failure. Methods Totally 34 SD rats were randomly divided into three groups: heart failure group(HF),treatment group(NRG) and sham operation group(S).Rat models of HF and NRG were made by aortocaval fistula(ACF).Rats in S group underwent aortic puncture instead of ACF.Eight weeks after ACF,rats in NRG group were injected with neuregulin-1β by tail vein,10 μg/(kg·d) for 7 days.Rats in HF and S groups were injected with the same dose of saline.Twelve weeks after operation,hemodynamic and echocardiogrophic characteristics and plasma BNP level were evaluated.The pathological lesions of myocardial tissues were evaluated after HE staining.And myocardial cell ultrastructure was investigated. Results ① Echocardiogrophic characteristics results showed that LVEF and LVFS were higher while LVESD and LVEDD were smaller in NRG group than in HF group(all P<0.05).② Hemodynamic results demonstrated that LVESP and ±dP/dtmax in NRG group were increased significantly and LVEDP was decreased significantly compared with those in HF group(all P<0.05).③ BNP level was markedly lower in NRG group than in HF group(P<0.01).④ Light microscopic and transmission electron microscopic examinations showed that damage to cardiomyocytes was improved by neuregulin-1β treatment compared with those in HF group. Conclusion Neuregulin-1β can obviously improve cardiac function and attenuate left ventricular damage in the volume-overloaded rat models with heart failure.
