实时超声显像LUNDH试餐冰棒法在胆囊收缩功能检查中的应用价值

摘　　要：目的 探讨实时超声显像LUNDH试餐冰棒法在胆囊收缩功能检查中的应用价值。方法 选择空腹血浆胆囊收缩素(CCK)水平正常,超声检查胆囊大小、形态正常、囊内胆汁透声好,油煎鸡蛋脂餐试验胆囊最大收缩率≥35%的50名受试者。试验前所有受试者均空腹行常规超声检查。试验1:选择体重50～60kg的23名受试者,第1天空腹服用LUNDH试餐冰棒1份(1支冰棒),同一组受试者于LUNDH试餐试验后第3天,空腹服用油煎鸡蛋1枚;试验2:选择体重50～100kg的受试者50名,分别于第1天、第3天、第5天空腹服用LUNDH试餐冰棒1份、1.5份、2份。所有受试者均分别于餐后15、30、60、90、120、150、180min行常规超声检查。常规超声检查测量胆囊最大长径、最大横径和最大前后径,计算每名受试者空腹胆囊容积、服用试餐后各个时间点胆囊容积,并取胆囊容积最小值作为胆囊最小残余容积,计算胆囊最大收缩率。结果 试验1:23名受试者服用LUNDH试餐冰棒后90min内均达到胆囊最大收缩率,而服用油煎鸡蛋后90min内仅有82.61%(19/23)达到胆囊最大收缩率。23名受试者服用LUNDH试餐冰棒1份所诱发的胆囊最大收缩率为(55.57±8.34)%,大于服用油煎鸡蛋1枚所诱发的胆囊最大收缩率(35.18±7.39)%,差异有统计学意义(t=8.777,P<0.05)。试验2:体重50～70kg的28名受试者服用1份试餐冰棒可达到胆囊最大收缩率,体重70～80kg的15名受试者服用1.5份试餐冰棒可达到胆囊最大收缩率,体重90～100kg的7名受试者服用2份试餐冰棒可达到胆囊最大收缩率。不同体重的受试者达到胆囊最大收缩率所需要的LUNDH冰棒试餐量随受试者体重的增加而增加。结论 LUNDH冰棒试餐制作、使用方便,便于存储,可作为标准脂餐用于胆囊收缩功能测定。实时超声显像LUNDH试餐冰棒法是理想的胆囊收缩功能检查方法,具有较好的临床应用价值。Objective To investigate the application values of real-time ultrasound and the ice candy of LUNDH test meal in checking the gallbladder contractive function.Methods Fifty volunteers were selected whose physical conditions met the following requirements:empty stomach,normal plasma cholecystokinin level,normal gallbladder size and shape in ultra inspection,bile in good entrant sound condition,biggest shrinkage rate of gallbladder≥35%.Before the test,all the volunteers are required to take routine ultrasound examination.(1)Twenty-three volunteers(50-60 kg)had one ice candy of LUNDH test meal on an empty stomach on the first day.On the third day,they had one fried egg.(2)Fifty volunteers(50-100 kg)had one ice candy of LUNDH test meal on an empty stomach on the first day,one and a half on the third day,and two on the fifth day.The volunteers took routine ultrasound examinations 15,30,60,90,120,150,180 minutes after the meal separately.Through routine ultrasound examinations,gallbladder maximum length diameter,maximum transverse diameter and maximum anteroposterior diameter were measured.Then,the volumes of gallbladder were calculated on an empty stomach as well as after each time point.The minimum volume of gallbladder was made the minimum residual volume.And biggest contraction rate of gallbladder was calculated.Results (1)All of the 23 volunteers having one ice candy of LUNDH test meal reached the maximum contraction rate in 90 minutes,while only 82.61%(19/23)of the volunteers having one fried egg reached the maximum contraction rate.The maximum contraction rate of gallbladder caused by taking one ice candy of LUNDH test meal was higher than that caused by taking a fried egg[(55.57±8.34)% vs(35.18±7.39)%]and the difference were statistically significant(t=8.777,P<0.05).(2)The 28 volunteers weighing between 50 to 70 kg can reach the maximum contraction rate by taking one ice candy of LUNDH test meal.The 15 volunteers weighing between 70 to 80 kg can reach the maximum contraction rate by taking one and a half.And
