肺外性、外伤性ARDS的影像学表现(附11例分析)

摘　　要：目的:总结肺外性、外伤性ARDS的发病率及影像学表现,以对此类患者在ARDS的发生及发展有比较准确的诊断,为临床治疗ARDS提供准确及时的影像诊断材料与方法:检查设备使用美国柯达DR,德国西门子HQCT,XGY 0.35T MRI。均拍胸部正位X线片、胸部CT平扫、头颅MRI平扫。诊断标准使用中华医学会呼吸病学分会1999年诊断标准。结果:肺外性外伤患者合并ARDS为1.1%。X线胸片表现:肺纹理加重并见边缘模糊或小斑片状影,较早期肺外带病变比内带严重。中期肺部可出现斑片状和大片状融合病变,后期病变严重时呈白肺表现。胸部CT扫描表现:肺内阴影从腹侧向背侧逐渐加重,呈仰卧位"沉降"区与肺后背区域。后期为大片状融合病变呈白肺表现。3例头颅MRI平扫表现:表现为缺血缺氧性脑病改变结论:ARDS病因可分为8市源性和肺外性的,肺泡上皮损伤使表面活性物质生成障碍,导致血液氧分压下降,发生急性呼吸窘迫综合征。根据X线胸片表现、胸部CT平扫的特征性表现,对肺外性、外伤后合并成人ARDS的患者,在医学影像方面能有较早期及准确的诊断。Purpose:Explore extrapulmonary sex,the incidence of merger ARDS trauma and imaging findings,to these patients in ARDS happening and development has more accurate diagnosis of,on the clinical ARDS with accurate and timely diagnosis.Materials and Methods:Check the equipment using U.S.kodak DR,HQCT,Siemens,a German XGY 0.35 T MRI.All clap chest is a X-ray,chest CT flat head MRI flat Saul,Saul.Of the association diagnostic criteria used breathing epidemiology branch diagnostic criteria in 1999.Results:Pulmonary sexual trauma merger ARDS 1.1%.X-ray chest radiograph performance:lung texture aggravating and sees the edge blur or small patchy shadow,lung periphery earlier than inside take serious disease.Medium-term lung can appear patchy and big flake fusion lesions,late in white lung disease is severe performance.Performance:chest CT scans from abdomen in lung shadow lateral dorsal progressive,is supine 'settlement ' area and lung back area.In the late great flake fusion lesions are white lung performance.3cases head MRI flat performance: performance for ischemia Saul encephalopathy change broadened.Conclusion:ARDS cause can be divided into the FeiYuanXing and pulmonary sex aleolar epithelial injury,make active surface substance obstacles,blood oxygen partial pressure drop,happen acute respiratory distress syndrome.According to the X-ray chest radiograph performance,chest CT flat on the performance,characteristic Saul pulmonary sex,trauma patients alter merger adult ARDS in medical imaging,could have a early and accurate diagnosis.
