摘　　要：目的:分析2002-2009年上海市不同等级公立医疗机构的经济运行情况。方法:收集2002-2009年卫生财务年报数据,进行描述性分析。结果:公立医疗机构三大收入来源仍然是药品收入、医疗收入和政府财政投入,在2007年出现收不抵支的情况,2008年、2009年实现盈余。政府财政投入尤其是对社区卫生服务机构投入有所增长;医疗收入中护理、诊察、治疗、床位占比逐渐降低,手术、化验、检查占比升高;药品加成率下降,但药品费增长快。人员支出增长速度低于总支出的增长速度,人员支出占总支出比例下降。结论:建议完善财政投入机制,合理调整医药价格,加强对医院的财务监管,整体推进公立医院改革。Objective: To analyze the economic status and government input of public hospitals at different levels from year 2002 to 2009.Methods: Health financial annual report during 2002-2009 in Shanghai was collected and a descriptive analysis was made.Results: The main sources of income for public health institutions were drug revenue,medical services revenue and government input.The balance of public hospitals turned into deficit in 2007,and achieved surplus in 2008 and 2009.The government input,especially for community health services institutions has increased.Among the medical services revenue,the proportion of nursing,curing,treatment and beds has decreased,but the proportion of surgery,laboratory and examination went higher.The growth rate of personnel expenditure was lower than that of total expenditure,leading to the lower proportion.Conclusion: It is suggested to improve the mechanism of government investment,appropriately adjust health care services prices and drug prices,improve the financial and accounting management system and promote the reform on public hospitals.
