浅谈胃切除术后胆囊收缩功能的变化

摘　　要：目的:探讨分析胃切除术后胆囊收缩功能的变化。方法:选取2007年10月～2009年10月间我院收治的41例接受胃切除手术的患者作为研究对象,并将其设为观察组,同时选取本院同期收治的未经手术治疗也无胆囊疾病的41例患有胃十二指肠疾病的患者作为对照组,将两组患者胆囊收缩功能的变化情况进行对比,并将对比结果及两组患者的临床资料进行回顾性的分析。结果:经随访,在41例观察组患者中,发生胆囊收缩功能减弱的患者有38例,发生胆囊收缩功能减弱的比率为92.68%,发生胆囊收缩功能不良的患者有36例,发生胆囊收缩功能不良的比率为87.80%。在41例对照组患者中,发生胆囊收缩功能减弱的患者有13例,发生胆囊收缩功能减弱的比率为31.71%,发生胆囊收缩功能不良的患者有10例,发生胆囊收缩功能不良的比率为24.39%。观察组患者胆囊收缩功能减弱的发生率及胆囊收缩功能不良的发生率均明显高于对照组患者,差异显著(p<0.05),具有统计学意义。结论:在胃切除术后,受术者的胆囊收缩功能明显减弱。此外,由于受术者的迷走神经肝胆支在术中被切断,因此导致其胆囊收缩无力、收缩素分泌减少,从而极易造成胆汁郁积淤滞,大大增加了胆囊结石的发生率。objective: to study the gastric resection of gallbladder contraction function change and gallstone disease. Methods: the October 2007 to October 2009 were during the stomach after resection of gallbladder stone surgery patients in 41 cases set to observe group, select the operation of our 41 cases without surgery nor gallbladder disease in patients with duodenal stomach 41 cases for control group, compared between the two groups of patients with gallbladder contraction function changes. Results: the follow-up observation, the observa- tion group happen gallbladder contraction function abate and an example, gallbladder contraction function abate rate was 92.68%, happen gallbladder contraction function bad, 36 patients with gallbladder contraction function bad rate was 87.80%; Control group happen gallbladder contraction function abate 13 cases, gallbladder contraction function abate rate was 31.71%, happen gallbladder contraction function bad 10 cases, gallbladder contraction function bad rate was 24.39%. Two groups of patients with gallbladder contraction function abate incidence and gallbladder contraction function on the incidence of adverse all have significant difference, p < 0.05, there is statistical significance. Conclusion: after gastrectomy, due to cut off the vagus nerve courage branch, gastrointestinal reconstruction, makes the gallbladder contrac- tion caused by weakness, cholecystokinin is secreted decrease and other reasons, the cholestasis clogged, gallbladder gallstone increased incidence.
