摘  要:目的探讨卵巢过度刺激综合征高危患者行全胚胎冷冻后首次移植对临床结局的影响。方法回顾性分析2010年1月~12月在我院生殖医学中心行超促排卵(COH)体外受精且并发OHSS高危风险190个周期的临床资料,其中新鲜胚胎移植周期118例(A组)、全胚胎冷冻后首次移植周期72例(B组)。比较两组患者COH情况、胚胎种植率及临床妊娠率。结果 B组HCG日雌激素水平及总优胚数(6171.40±1978.92,7.78±4.44)高于A组(4949.50±1391.66,6.20±3.45),差异有统计学意义(P<0.05);B组的临床妊娠率高于A组、种植率低于A组,但差异无统计学意义(P>0.05);A组中有一例患者发生了迟发型OHSS。结论全胚胎冷冻后首次移植可避免迟发型OHSS的发生;且并不影响OHSS高危患者的临床结局,是一种安全而有效的方法。Objective To explore the clinical outcomes of all frozen embryo transplantation in patients at risk of ovarian hyperstimulation syndrome(OHSS).Methods A retrospective analysis was made in the clinical data of 190 patients at high risk of OHSS after controlled ovarian hyperstimulation between January 2010 to December 2010 in our reproductive center.including 118 fresh embryo transfer cycles(group A),and 72 all frozen embryo transfer cycles(group B),The controlled ovarian hyperstimulation(COH),implantation rate and clinical pregnancy rate were compared between group A and group B.Results The total number of high-quality embry os and the levels of estrogen on HCG day in group B(6 171.40±1 978.92,7.78±4.44) showed a significantly higher than those in group A(4 949.50±1 391.66,6.20±3.45)(P<0.05);Compared with group A,the clinical pregnancy rate were slightly but not significantly higher in group B,while the implantation rate is not significantly lower in group B;A patient occurred late OHSS in group A.Conclusion All frozen embryo transplantation might prevent the risk of late OHSS in patients undergoing IVF treatment and does not affect clinical outcomes in patients at risk of OHSS,which is a safe and effective method.
