干扰素治疗乙型肝炎外周血中性粒细胞动力学变化分析

摘　　要：目的探讨α干扰素治疗慢性乙型肝炎患者外周血中性粒细胞动力学的变化。方法对接受α干扰素治疗期间出现中性粒细胞计数(NEU)﹤1.0×109/L的患者进行骨穿检查,肾上腺素及强的松激发实验,检测NEU﹤1.0×109/L的患者在治疗前后的粒细胞集落刺激因子(G-CSF)水平的变化。结果共有32例患者在α干扰素治疗期间出现NEU﹤1.0×109/L,占44.4%,全部病例骨穿检查均不存在骨髓抑制;29例患者肾上腺素激发试验阴性;27例患者强的松激发试验阴性;治疗前与NEU﹤1.0×109/L时G-CSF水平分别是(161.26±2.53)pg/ml与(101.6±2.56)pg/ml,P﹤0.05。结论α干扰素治疗期间外周血粒细胞下降与内源性G-CSF水平的下降有关,应适时补充外源性G-CSF。OBJECTIVE To explore dynamics of neutrophils peripheral blood changes during the trentment of interferon therapy in patients with chronic hepatitis B.METHODS Patients who received interferon therapy with netrophil count﹤1.0×109/L received bone marrow puncture and smear,adrenaline and prednisone stimulate test,and changes of neutrophils stimulating factor(G-csf)level of patients with NEU﹤1.0×109/L before and after treatment were tested.RESULTS A total of 32(44.4%)patients appeared NEU﹤1.0×109/L.All cases did not existed bone marrow inhibition.29 patients showed negative by the adrenaline proocation test,and 27 patients showed negative by the prednisone proocation tests,and G-csf levels were(161.26 ± 2.53)pg/ml and(101.6 ± 2.56)pg/ml,respectively,P﹤0.05.CONCLUSION The neutropenia during interferon alpha treatment is related to decreased endogenesis G-csf level,which should be timely added exogenous G-csf.
