56例老年肺部韦格氏肉芽肿患者的CT影像学特征分析

摘　　要：目的回顾性分析老年Wegener肉芽肿肺部CT影像学特征,以提高影像诊断的准确性。方法 56例病理证实的老年肺部Wegener肉芽肿,均经CT平扫和增强扫描检查,对CT影像资料进行回顾性分析,提高对本病CT征象的认识。结果 42例表现为肺内多发性病灶,其中35例为结节性,7例为斑片状,余14例为肺孤立性灶。35例病灶内可见空洞,增强扫描多数呈边缘性强化,其内坏死区无强化。结论肺Wegener肉芽肿的CT扫描主要表现为多发的胸膜下结节、肿块及楔形实变,结节性内空洞或浸润性病变以及病变周边强化。CT对肺Wegener肉芽肿具有重要的诊断意义。OBJECTIVE To analyze the elderly Wegener's granulomatosis lung CT imaging characteristics,in order to improve the accuracy of diagnostic imaging.METHODS 56 cases of pathologically confirmed elderly patients with pulmonary Wegener's granulomatosis,were confirmed by CT scan and enhanced scan,the CT image data were retrospectively analyzed to improve the understanding of the CT signs of the disease.RESULTS 42 cases presented the multiple lesions of lungs,of which 35 cases were nodular,seven cases were patchy,and 14 cases were solitary pulmonary lesions.Lesions of 35 cases seemed empty,enhanced scan showed edge enhancement,and necrosis did not show enhancement.CONCLUSION CT scans of pulmonary Wegener's granulomatosis mainly showed multiple subpleural nodules,lumps and wedge-shaped consolidation,nodular empty or invasive lesions and lesions of peripheral enhancement.Pulmonary CT has important diagnostic significance for Wegener's granulomatosis.
