应用微型营养评价精法评价老年慢性阻塞性肺疾病患者的营养状况

摘　　要：目的应用微型营养评价精法(short-form mini-nutritional assessment,MNA-SF)评价老年阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者的营养状况,并比较不同营养状况患者的传统营养评价指标的差异。方法采用MNA-SF对68例老年COPD患者进行营养评价和传统营养指标测定并进行分析。结果老年COPD患者营养不良、营养不良危险发生率分别为35.29%和19.12%,而营养正常患者只占45.59%。对3种不同营养状况患者的传统营养指标分析后发现:3组患者的体质量指数(body mass index,BMI)、三头肌皮褶厚度(triceps skin-fold,TSF)、上臂肌围(arm muscle circumference,AMC)、小腿围(canna circumference,CC)、白蛋白(al-bumin,ALB)、前白蛋白(prealbumin,PAB)、总胆固醇(total cholesterol,TC)、三酰甘油(triglyceride,TG)、血红蛋白(hemoglobin,HGB)、总淋巴细胞(total lympha cell,TLC)差异有统计学意义(P<0.05),而TC差异无明显统计学意义。3组患者之间两两比较后,BMI、TSF、ALB、PAB均有显著差异(P<0.05),营养正常组>营养不良危险组>营养不良组;营养正常组AMC、CC、TG、HGB、TLC明显高于营养不良组(P<0.05);营养不良危险组TG明显高于营养不良组(P<0.05)。结论老年COPD患者营养不良和营养不良危险的发生率较高,其传统营养评价指标也会下降。Objective To evaluate the nutritional state of aged patients with chronic obstructive pulmonary disease(COPD),and to compare difference of the traditional assessment index among patients with different short-form mini-nutritional assessment(MNA-SF) scores.Methods MNA-SF was used to assess the nutritional state of 68 aged patients with COPD,and the traditional nutritional assessment index was analyzed.Results Among the aged inpatients with COPD,the incidence of malnutrition,the risk of malnutrition and good nutritional status were 35.29%,19.12% and 45.59% respectively.There were significant difference in the body mass index(BMI),triceps skin-fold(TSF),arm muscle circumfe-rence(AMC),canna circumference(CC),albumin(ALB),prealbumin(PAB),total cholesterol(TC),triglyceride(TG),hemoglobin(HGB)and total lympha cell(TLC) among the three groups(P<0.05).The levels of BMI,TSF,ALB and PAB were significantly different in pairwise comparison.Conclusions Incidence of malnutrition and the risk of malnutrition of the aged patients with COPD were comparatively high,and the level of their traditional nutrition assessment index woud descend.
