摘  要:目的探讨血清氧化低密度脂蛋白抗体IgM(ox-LDL-AbIgM)水平在急性ST段抬高心肌梗死(STE-MI)患者中与冠脉病变程度及丙二醛(MDA)、血脂的相关性。方法诊断明确并行急诊PCI治疗的STEMI患者95例,根据冠脉造影结果分为单支病变组、双支病变组及3支病变组,分别测定血清ox-LDL-AbIgM、MDA、血脂,计算体质量指数(BMI),分析ox-LDL-AbIgM水平与冠脉病变程度及MDA、血脂的关系。结果 STEMI患者血清ox-LDL-AbIgM水平随冠状动脉病变支数增加而降低(P<0.05);ox-LDL-AbIgM水平与血脂、血糖水平等未见明显相关性(P>0.05),与MDA水平呈显著负相关性(r=-0.319,P<0.05)。结论血清ox-LDL-AbIgM水平可在一定程度上反映STEMI患者冠状动脉病变的严重程度;它可能通过调节机体氧化应激水平,减少LDL氧化修饰生成ox-LDL,从而减轻冠状动脉粥样硬化的面积。Objective To examine the association between serum autoantibodies against oxidized low-density lipoprotein IgM (ox-LDL-AbIgM) and coronary lesions as well as malonaldehyde(MDA) and lipids in patients with ST-segment elevation acute myocardial infarction (STEMI). Methods Ninety-five patients with STEMI who underwent elective coronary angiography (CAG) and PCI were enrolled and divided into single-, doubleand triple-vessel lesion group according to the number of vessels with significant stenosis.Ox-LDL-AbIgM, MDA, serum lipids, body mass index (BMI) in 3 groups were examined and analyzed. Results There were significant differences in ox-LDL-AbIgM level among different vessel disease groups(P<0.05). Person correlations showed Ox-LDL-AbIgM was negatively and significantly correlated with MDA (r=-0.319, P<0.05), but was not correlated with other clinical factors (P>0.05). Conclusions The level of ox-LDL-AbIgM can reflect the severity of atherosclerosis in patients with STEMI; ox-LDL-AbIgM may paly a role in adjusting the oxidative stress to inhibil LDL being modificated to ox-LDL, and reduce the coronary atherosclerosis area.
