老年高血压患者高血压类型及分级与动脉波传播速度关系研究

摘　　要：目的探讨不同程度老年高血压患者臂-踝脉搏波传导速度(baPWV)的影响因素以及不同血压类型对baPWV的影响。方法老年高血压患者120例,依据血压水平分为单纯收缩期高血压(ISH)及单纯舒张期高血压(IDH),ISH组患者根据高血压1、2、3级分成A、B、C组,IDH组分成Ⅰ、Ⅱ、Ⅲ组,52例血压正常的老年患者为对照组(D组),对所有患者进行baPWV检测,分析不同收缩压(SBP)水平或者舒张压(DBP)水平间baPWV的差异,采用多元逐步回归法确定baPWV的影响因素。结果 ISH各组baPWV、SBP、脉压比较差异均有统计学意义(P<0.05),IDH各组baPWV、DBP比较差异均有统计学意义(P<0.05),逐步回归分析结果显示ISH组baP-WV与年龄、SBP、脉压相关,IDH组baPWV与年龄相关。结论年龄、SBP、脉压是老年高血压患者baPWV的主要决定因素,早期控制血压有助于改善老年高血压患者baPWV。Objective To investigate the risk factors of brachial-ankle pulse wave velocity (baPWV) and the relationship of blood pressure type with baPWV in the elderly hypertensive patients. Methods A total of 120 elderly hypertensive patients were divided into isolated systolic hypertension (ISH) group and isolated diastolic hypertension (IDH) group.The patients in ISH group were divided in group A, B, C and the patients in IDH group were divided into group Ⅰ, Ⅱ, Ⅲ according to hypertension 1,2,3 level.Fifty-two elderly cases with normal blood pressure were enrolled as control group (group D). The level of baPWV was detected in all patients. The difference in the level of baPWV was analyzed between the patients with different SBP level or DBP level.The risk factors of baPWV were determined using multiple regression. Results The levels of baPWV, systolic blood pressure, pulse pressure showed significant differences between the ISH groups (P<0.05), and the levels of baPWV and diastolic blood pressure showed significant differences between IDH groups (P<0.05). Stepwise regression analysis showed that the level of baPWV was related with age, systolic blood pressure, pulse pressure in ISH group, and related with age in IDH group. Conclusions Age, systolic blood pressure, pulse pressure are the main risk factors of baPWV in the elderly patients with hypertension. Controlling the systolic blood pressure and pulse pressure can help improve baPWV.
