摘  要:目的探讨彩色多普勒超声诊断肾细胞癌及术前分期的临床应用价值。方法对112例拟诊肾细胞癌患者进行彩色多普勒超声检查,以Robson分期为基础,结合TNM分期方法进行术前分期,并与手术、病理诊断结果进行对比。结果 112例肾细胞癌中,超声诊断正确106例,符合率为94.6%(106/112);超声诊断不正确6例(5.4%,6/112),其中误诊为错构瘤及囊肿感染各2例,漏诊2例;超声检查术前分期正确96例,符合率为90.6%(96/106),其中T4期最高100%(13/13),T1期次之94.4%(34/36);T2期和T3期相对较低,分别为84.5%(28/33)与87.5%(21/24)。超声检查术前分期不正确10例[9.4%(10/106)]。结论彩色多普勒超声诊断肾细胞癌及术前分期的准确率均较高,具有重要的临床应用价值。Objective To explore the clinical value of color Doppler ultrasound in diagnosis and preoperative staging of renal cell carcinoma(RCC).Methods A total of 112 suspected RCCs were examined by gray scale ultrasound and color Doppler flow imaging.Preoperative staging was made based on Robson and TNM classifications,and was compared with postoperative pathological results.Results Among the 112 cases,106 cases were diagnosed as RCC.The diagnostic accuracy rate of color Doppler ultrasound was 94.6%(106/112).Six cases are misdiagnosed,accounting for 5.4%(6/112).Of them,two cases were misdiagnosed as hamartoma,two cases as infective cyst and two cases were missed.The accuracy rate of preoperative staging by color Doppler ultrasound was 90.6%(96/106).In detail,it was 100%(13/13)for T4 stage,94.4%(34/36)for T1 stage,84.5%(28/33)for T2 stage and 87.5%(21/24)for T3 stage respectively.The preoperative staging was incorrect in 10 cases,accounting for 9.4%(10/106).Conclusion Given the high accuracy in diagnosis and preoperative staging of RCC,color Doppler ultrasound could be of great value in clinical application.
