针刺配合康复训练对脑卒中软瘫期上肢肌张力及运动功能恢复的影响

摘　　要：目的观察针刺配合康复训练对脑卒中后软瘫期上肢肌张力及运动功能的影响,为脑卒中后康复训练和功能预测提供依据。方法将60例发病两周内的脑卒中偏瘫迟缓期的入院患者随机分为观察组(康复训练结合针刺)和对照组(针刺)。采用修订的Ashworth痉挛量表评定两组患者治疗两周后的肌张力。采用Fugl-Meyer评分法(FMA)评定两组患者治疗前后上肢运动功能。临床疗效评定采用Brunnstrom分期法。结果观察组与对照组总有效率比较有显著性差异(P<0.05),观察组治疗后上肢运动功能的恢复优于对照组并且处于软瘫期的病例数少于对照组(P<0.05)。结论针刺结合康复训练有助于早期诱发软瘫期上肢肌张力的出现,缩短脑梗死软瘫期,并可显著改善上肢运动功能,减少致残率。Objective To observe the influence of acupuncture combined with rehabilitation training on upper limb muscle tension and motor function in flaccid paralytic period,and provide a basis of rehabilitation training and prediction of motor function of limbs after stroke.Methods 60 patients of flaccid paralytic period(less than 2 weeks) were randomly divided into observation group(acupuncture combined with rehabilitation training) and control group(acupuncture).The two groups after two weeks treatment in Modified Ashworth was used to assess the Muscle tension.The upper limb motor function of two groups was evaluated by Fugl-Meyer Assessment before and after treatment.The clinical efficacy was evaluated with Brunnstrom staging method.Results The total effective rate of observation group and control group have significant difference(P < 0.05).Upper limb motor function recovery after treatment in observation group is better than control group,and the number of flaccid paralysis patients in the observation group is significantly less than that in the control group(P < 0.05).Conclusion Acupuncture combined with rehabilitation training help early induced occurrence of upper limb muscle tension of flaccid paralysis period.It also can shorten the period of flaccid paralysis,greatly improve the limb motor function and reduce the disability rate.
