药物流产不全早期预测及相关因素分析

摘　　要：目的探讨早期预测药物流产不全的指标,并对其相关影响因素进行调查分析。方法对使用药物终止妊娠的196例健康妇女,在孕囊排出后当天使用经阴道彩色多普勒超声检测着床部位的血流信号和该部位螺旋动脉阻力指数(RI)值,追踪其药物流产结局,进而选择早期预测药物流产不全的指标。再对358例药物流产妇女经检测孕囊排出后超声表现获得流产不全高危者60例,对其相关因素进行调查分析,探讨药物流产不全高危的影响因素。结果 196例研究对象中不完全流产29例,其孕囊排出后当天着床部位仍显示明显的血流信号,27例RI值≤0.6。孕囊排出后RI值≤0.6提示药物流产不全高危的准确率为96.43%。药物流产不全高危病例的存在与产次、人工流产次数、胚芽情况及盆腔炎症病史等因素密切相关。结论孕囊排出后着床部位呈现RI值≤0.6的血流信号可预测药物流产不全的发生,此高危情况的存在与产次、人工流产次数、胚芽情况及盆腔炎症病史等因素密切相关。Objective To discuss the sensitive index of incomplete drug abortion forecasted in early stage,and to investigate and analyze the related influence factors.Methods For the 196 cases of healthy women who adopt drugs to terminate pregnancy,the transvaginal color Doppler sonography was used to test the blood signal of the gestational sac implantation site and its spiral arterial resistance(RI) index after the gestational sac discharge,observe the result of the drug abortion so as to select best index of incomplete drug abortion forecasted in early stage.Then 358 cases of healthy women who underwent drug abortion routinely were tested by the blood signal and RI value after the gestational sac discharge,and obtain 60 high-risk cases.The related factors of them were investigated and analyzed and the influence factors of incomplete drug abortion high-risk cases were discussed.Results 29 cases of incomplete drug abortion were detected and there were obvious blood signal of the gestational sac implantation site after the gestational sac discharge.Among them,27 cases with RI value≤ 0.6 after the gestational sac discharge suggested that it was highly possible of incomplete drug abortion.The forecast accuracy rate is 96.43%.The existence of high-risk cases is closely related to parity,times of artificial abortion,germ situation and medical history of pelvic infection disease,etc.Conclusion Obvious blood signal with RI value≤0.6 of the gestational sac implantation site after the gestational sac discharge can forecast the incomplete drug abortion.The high-risk condition is closely related to parity,times of artificial abortion,germ situation and medical history of pelvic infection disease.
