我国基本医疗服务公平及效率问题研究

摘　　要：通过对当前基本医疗服务公平及效率问题进行分析,以充分利用卫生资源,实现最佳的社会效益和经济效益。通过对当前基本医疗费用支出、医疗费用负担情况、卫生总费用机构流向情况、医疗服务情况及农村三级医疗卫生服务的基本情况,浙江省乡镇卫生院人力资源情况、影响群众就医因素等进行调查分析,提出当前我国基本医疗服务存在着国家卫生政策的导向与实际给予的投入不相匹配、城乡卫生服务的差异在不断扩大、基本医疗中仍集中在治疗而不重视预防工作及基本医疗服务存在着不平等性等问题。并建议必须要建立健全可持续的财政支持机制,保证更多的财政资金投向农村医疗卫生等公共服务领域,增强城乡居民的健康意识,强化基本医疗服务的均等化等方式,提升我国基本医疗服务的效率。Through analyzing efficiency and fairness of the current basic medical service,health resources will be made full use of,and the best social benefit and economic benefit will be achieved.Based on the current China's basic medical expenses,medical expenses burden,direction of flow to institutions about the total health expenses,medical service situation,three-level medical health service in rural areas,structure of staff about health clinics in towns and townships,and the factors which influences people to see a doctor are investigated,and it is proposed that the guide of a national health policy doesn't match the actual input,the gap of medical service are growing between urban and rural town the focal point of the basic medical isn't on prevention and there is unfairness of the basic medical service.It is suggested that sustainable financial support mechanism should be established and perfected,more funding should be poured to rural health care and other public services,health consciousness of urban and rural residents should be strengthened,basic medical service should be equalized,and the efficiency of basic medical services in China should be improved.
