颈部舌下神经鞘瘤的超声表现分析

摘　　要：目的探讨舌下神经鞘瘤的彩色多普勒超声表现及其诊断价值。方法回顾性分析3例经手术病理证实的颈部舌下神经鞘瘤患者的彩色多普勒超声表现及其与周围组织的关系。结果颈部舌下神经鞘瘤的超声表现与其他神经来源的颈部神经鞘瘤一样,表现为肿瘤包膜完整,内部呈低回声且分布均匀,边界清晰,部分可在肿瘤长轴端看见梭形高回声,肿瘤内血流不明显或仅有稀疏血流。3例患者肿瘤均位于颈总动脉分叉水平以上,颈内动脉、颈外动脉分别位于肿瘤内外侧,肿瘤紧贴颌下腺,位于颌下腺的外侧或下方,在二腹肌上方。结论了解舌下神经鞘瘤彩色多普勒超声表现及其与周围的解剖关系,可提高超声对舌下神经鞘瘤诊断的准确率。Objcetive To investigate the value of color Doppler ultrasound in diagnosis of hypoglossal neurilemmoma(HGN).Methods Three cases with HGN confirmed pathologically were selected.The diagnostic performance characteristics of color Doppler ultrasound,as well as the relationship between HGN and surrounding tissues was retrospectively analyzed.Results The ultrasonic manifestations of HGN were the same as other cervical neurilemmoma.Three cases presented in B-mode ultrasonography was solid,well defined,hypoechoic and homogeneous,with fusiform hyperecho in the long axis sometimes.The tumor blood flow was not obvious or only sparse.Three HGNs were located superior to the carotid bifurcation,between internal carotid artery and external carotid artery,lateral or inferior to the submandibular gland,and superior to the biventer mandibulae.Conclusion Color Doppler ultrasound may improve the diagnostic accuracy of HGN by combination of color Doppler flow imaging(CDFI) performance of HGN and the relationship between HGN and surrounding tissues.
