高频超声及X线钼靶在社区妇女普查中诊断早期乳腺癌的价值

摘　　要：目的比较高频超声、X线钼靶在社区妇女普查中诊断早期乳腺癌的优缺点,探讨两者联合诊断的价值。方法选择2008年11月-2011年6月进行妇女乳腺癌筛查并经手术病理学证实的23例早期乳腺癌(TisN0M0和T1N0M0)患者的影像资料进行回顾性分析。结果高频超声诊断符合率为87.0%(20/23),表现为实性占位16例,3例伴针尖样钙化,15例可见血流信号,4例为低回声区。X线诊断符合率为69.6%(16/23),表现为肿块影14例,边缘模糊及伴毛刺者5例,有小分叶3例,有毛刺及血管征者3例,合并钙化影者5例。1例局部未见肿瘤影,1例表现为微小钙化。高频超声、X线钼靶联合诊断与病理诊断符合率为91.3%(21/23)。结论高频超声在社区妇女乳腺普查中对早期乳腺癌的发现有重要价值,联合X线钼靶可进一步提高早期乳腺癌的诊断准确率。Objective To compare the advantages and disadvantages of high frequency ultrasound and mammography in diagnosis of early stage breast cancer in community women.Methods A total of 23 female cases with breast cancer in our hospital from November 2008 to June 2011 were selected.The cases received high frequency ultrasound and mammography examination before operation and were confirmed by surgical pathology as early breast cancer(TisN0M0 and T1N0M0).Their data were retrospectively analyzed.Results The diagnosis coincidence rate of high frequency ultrasound was 87.0%(20/23),with solid mass in 16 cases,tip calcification in 3 cases,visible blood flow signal in 15 cases,and low echo area in 4 cases.The diagnosis coincidence rate of mammography was 69.6%(16/23),with mass shadow in 14 cases,fuzzy edge and burr in 5 cases,small leaves in 3 cases,burr and vascular shadow in 3 cases,calcification in 5 cases.Two cases showed no local tumor shadow,manifesting as microcalcification.The diagnosis coincidence rate of high frequency ultrasound combined with mammography was 91.3%(21/23).Conclusion In breast cancer screening in the community women high frequency ultrasound is valuable in discovery and diagnosis of early stage breast cancer.High frequency ultrasound combined with mammography can greatly improve the diagnosis of early stage breast cancer.
