纤维支气管镜检段支气管开口变异的观察分析

摘　　要：目的分析总结段支气管开口变异的规律,以指导临床实践。方法通过对580例纤维支镜受检者资料的回顾性分析,统计分析各种段开口变异的发生率。结果观察580例发现段支气管变异者90例。发生在右上叶者最为多见共42例,占7.24%,表现为从通常的3段变为2段。其次为左上叶之固有上叶共31例,占5.34%,表现为固有上叶从通常的2段变为3段。另外还有右中叶呈三段者10例,占1.72%。左舌叶单独开口于左主支气管者7,例占1.21%。结论通常情况下有15.52%的人发生段支气管开口变异,以右上叶最为多见,其次为左上叶,再其次为右中叶。认识段支气管开口正常变异在指导纤支镜检查时正确区分有无支气管阻塞及正确有认识肺的解剖结构有重要的指导意义。Objective The segmental bronchus opening's variations were determined and analysed in order to guide clinical practice.Method By retrospective analysis of 580 bronchofiberscope subject's materials,to determine the incidence of segmental bronchus opening's variation.Result There were 90 patients with segmental bronchus opening's variations in 580 patients.The most common was variant in right upper lobe(42 patients).It were of 7.24 percents.Its manifestation was that three segments changed to two segments.The second common was variation in left upper lobe,there were 31 patients(5.34 percent).Its manifestation was that two segments changed to three segments at inherent up leaf.Besides,the right middle lobe changed to 3 segments from 2 segments and lingual lobe alone opened at left primary bronchus.Conclusion Under usual circumstance,patients of segmental bronchus opening's variation were of 15.52 percent.The most common was variation of right upper lobe.The second was variation of left upper lobe.Realization of segmental bronchus opening's variation has important guiding significance to diagnose bronchus blockage and realize lung anatomic structure.
