摘 要:目的评价白细胞分化抗原CD14基因启动子上游-159C/T位点基因多态性与冠心病遗传易感性的关系。方法检索PubMed、Web of science、CNKI、万方、CBM数据库收录的2012年3月以前发表的关于CD14-159C/T基因多态性与冠心病相关性的病例对照研究论文,中/英文检索词:"冠心病/coronary disease/coronary artery disease/coronary heart disease,CD14/CD14,基因多态性/genetic polymorphism",对入选文献进行异质性检验后,将CD14-159C/T的基因型、等位基因按暴露组与非暴露组分别组合为CCvsCT+TT、TTvsCC+CT和TvsC多态性模型,计算不同组合多态性模型合并OR,采用RevMan5.0统计软件进行Meta分析。结果共7篇文献纳入研究,累计病例2005例,对照1433例。分析结果显示CC vs CT+TT的OR=0.64(P>0.05);TT vs CC+CT的OR=1.33(P<0.01);T vs C的OR=1.31(P<0.05)。剔除2篇非中国汉族人群研究,CC vs CT+TT的OR=0.39(P<0.01);TT vs CC+CT的OR=1.76(P<0.01);T vs C的OR=1.68(P<0.01)。结论 CD14启动子上游-159C/T位点基因多态性与冠心病易感性相关,T等位基因可能是冠心病的遗传易感基因。Objective To study the correlation between the CD14 gene promoter polymorphism-159C/T and coronary heart disease(CHD) by means of Meta-analysis. Methods All case-controll studies were searched,which were about correlation of CD14-159C/T polymorphism to coronary disease published before March 1st,2012 in the PubMed,Web of science,CNKI,Wanfang and CBM datebases.Search words to find appropriate studies were coronary heart disease,coronary artery disease,coronary disease,CD14 and genetic polymorphism.Publication bias was tested by funnel plot and the combined odd ratios(OR)then were calculated.Three polymorphous models of CC vs CT+TT、TT vs CC+CC and T vs C based on risk gene exposure group and non risk gene exposure group were supposed and used to calculate OR.The software Review Manager(version 5.0) was used for Meta-analysis. Results Seven appropriate studies were adopted.Integrated 2 005 cases and 1 433 controls from studies adopted were analyzed.The results showed OR of polymorphous models of CC vs CT+TT,TT vs CC+CT and allele T vs the allele C were respectively 0.64(P>0.05)and 1.33(P<0.01)and 1.31(P<0.05).After rejected two studies about different race,corresponding OR above three polymorphous models were 0.39(P<0.01),1.76(P<0.01)and 1.68(P<0.01)respectively. Conclusions It is indicated that the-159C/T polymorphism in CD14 gene promoter is significantly associated with increased risk of coronary disease,the allele T may be a genetic risk factor for coronary disease.
