动脉血乳酸水平及乳酸清除率变化对危重新生儿预后的评估意义

摘　　要：目的探讨新生儿入院24h内动脉血气中乳酸水平及乳酸清除率变化对危重新生儿预后的评估意义。方法回顾性分析78例危重新生儿,收集入院时和治疗6h后的动脉血乳酸值,根据预后不同分为存活组53例、死亡组(含放弃)组25例。比较乳酸水平与乳酸清除率的差异。结果存活组血乳酸水平5.92±2.75,乳酸清除率52.53%±8.32%,死亡组血乳酸水平为7.94±3.83,乳酸清除率为27.35%±6.89%,乳酸水平与乳酸清除率两组间差异均有统计学意义。结论血乳酸水平及乳酸清除率对危重新生儿预后判断有一定帮助。Objective To explore the significance of arterial blood gas lactic acid level and lactic acid clearance changes of neonatal on the prognosis of newborns within 24 hours of hospitaliation. Methods Data of 78 critically ill newborns collected on admission and treatment after six hours were retrospectively analyzed.According to different prognosis,the patients were divided into live group(53 patients) and death group(25 cases).The lactic acid levels and the lactic acid clearance were compared.Results The blood lactic acid levels of live group were 5.92±2.75,the blood lactic clearance rate were 52.53%±8.32%;in the death group,blood lactic acid level were 7.94±3.83 and blood lactic clearance rate were 27.35%±6.89%.The differences between the two groups were statistically significant. Conclusion The blood lactic acid level and lactic acid clearance would be helpful for the prognosis prediction of newborns.
