经闭孔阴道无张力尿道中段悬吊术治疗女性压力性尿失禁16例疗效观察

摘　　要：目的 探讨经闭孔阴道无张力尿道中段悬吊术(TVT-O)治疗女性压力性尿失禁的临床疗效。方法 回顾性分析2006-01～2013-06间在我院行经闭孔阴道无张力尿道中段悬吊术治疗女性压力性尿失禁16例患者临床资料。结果 16例患者平均手术时间(25.4±3.1) min,平均失血量(36.7±2.8)ml,平均医疗费用(9554.6±672.8)元,平均住院日(4.5±2.4)d。所有病例均获随访,时间12 ～24个月,平均(16.24±3.65)个月。术后治愈率达87.5％(14/16),改善率12.5％(2/16),术后排尿困难2例,腹股沟疼痛1例,经对症处理后缓解。结论 TVT-O治疗女性压力性尿失禁疗效显著,创伤小,操作简单,并发症少,但医疗费用相对较高。Objective To study the curative effect of Tension-free Vaginal Tape-Obturator procedure( TVT-O) for Female Stress Urinary Incontinence( SUI). Methods A retrospective study was undertaken to analyze the clinical characteristics of 16 cases with SUI from Jan. 2008 to Jun. 2012. Results The average operative duration was( 25. 4 ± 3. 1) minutes,average blood loss was( 36. 7 ±2. 8) ml,the average cost was( 9554. 6 ± 672. 8) yuan,and average hospital stay was( 4. 5 ± 2. 4) days. All cases had been followed up for12 ~ 24 months,with an average of( 24. 21 ± 1. 25) months. The curative rate was 87. 5%,and improvement rate was 12. 5%. There were2 cases of voiding dysfunction and one case of groin pain occurred postoperatively and all these cases were relieved by corresponding management. Conclusion TVT-O is an effective treatment for female SUI with advantages of simple operating,minimal invasion,safety,and few complications,but with the disadvantage of high cost.
