摘 要:目的探讨256层iCT灌注成像技术在高压电损伤后判断损伤肢体血管及肌肉微循环的变化,为临床诊断、治疗提供指导的意义。方法采用Phillips公司256层智能CT(intelligent CT,iCT)对所有患者进行双侧肢体及相应躯干同时灌注检查,所得图像传入Phillips公司专属EBW工作站,用Functional CT-general模式行灌注图像处理,计算灌注参数和重建相应的灌注参数图;所得结果与临床手术中探查结果、病理检查结果进行对比分析。结果 67例患者经MSCTP检查,均得到清晰血管CTA图像,CTA示血管狭窄、中断、闭塞11例,血管未见明显异常14例;肌肉损伤67例。行截肢手术11例,皮瓣修复手术31例,植皮手术35例。67例患者手术中均将间生态组织送检病理检查,结果示:炎性反应。结论 256层iCT高压电损伤肢体灌注成像,检查安全、方便,图像处理简单省时、图像质量清晰,能同时获得肢体血管形态信息及肌肉微循环信息,为临床治疗方式的选择提供了积极、有益的帮助。Objective To investigate the role of 256- slice CT perfusion imaging on high voltage electrical injuried limbs in displaying blood vessel and muscle- microcirculation. Methods Simultaneously,both- limbs and trunk were perfused by Phillips 256- iCT.Then,all the images were imported to EBW workstation,image manipulation were disposed by using Functional CT- general mode,perfusion parameter and perfusion picture were re- established too. The results of clinical surgery exploration and pathology results were analyzed.Results 67cases,all get clear CTA imaging.There were angiostegnosis,discontinue,block up of 11 cases,14 cases were normal.67 cases were muscle damaged.Amputation in 11 cases,flap surgery in 31 cases,35 cases of skin grafting.67 cases of organizational samples were sent to pathological examination.Conclusion 256- iCT perfusion is a safe,convenient, simple examination method,it can obtain the informention of blood vessel and muscle- microcirculation,it could provide much more and full- scale image data for clinical treatment.
