严重腹部闭合性损伤伴多发伤的急救体会

摘　　要：目的探讨严重腹部闭合性损伤伴多发伤患者的有效救治途径。方法对2005年1月~2011年1月我院急诊收治的87例严重闭合性腹部损伤伴多发伤的病例资料进行回顾性分析。结果 87例患者在急诊科进行紧急处理后均行急诊手术治疗,术中死亡3例,术后死亡8例,死亡率12.64%。结论严重腹部闭合性损伤伴多发伤患者在急诊科通过监测生命体征、氧饱和度的变化,行腹腔穿刺、床旁B超、X线等检查,及早对伤情做出快速、准确评估,及时实施有效的复苏,建立"绿色通道"等各项措施是争取抢救时间、降低死亡率、提高救治成功率的关键措施。Objective To study the effective methods of improving the diagnosis and treatment of severe blunt abdominal trauma with multiple injuries. Methods The clinical data of 87 cases with severe blunt abdominal trauma with multiple injuries hospitalized in our hospital from Jan 2005 to Jan 2011 were reviewed retrospectively. Results 87 cases underwent operative surgery after emergency treatment in the emergency department,3 cases of intraoperative death,8 cases of postoperative death,the mortality was 12.64%. Conclusion The key measures of secure the rescue time,reduce the mortality and improve the treatment success rate are to make fast,accurate assessment about injury,timely implementation of effective recovery,establish a 'green channel' and to monitor vital signs. Changes of oxygen saturation,and to do the abdominal puncture. Bedside B-ultrasound,X-ray examination,et al,in the emergency department about severe blunt abdominal trauma with multiple trauma patients.
