摘 要:目的探讨促红细胞生成素(erythropoietin,EPO)对大鼠腰神经根慢性压迫性损伤的保护作用。方法将54只SD大鼠随机分为生理盐水(NS)对照组、EPO硬膜外隙给药组、EPO尾静脉给药组,术后每组动物分别存活1、2、4周,不同时段各组动物均为6只。手术经大鼠左侧L5椎间孔插入中空钢管,对神经根形成稳定的压迫,造模术后NS对照组与EPO硬膜外隙用药组分别经PE管于硬膜外隙注射NS与EPO,EPO尾静脉给药组经尾静脉注射EPO,观察用药后大鼠行为学变化,应用HE染色观察脊髓神经细胞病理变化,免疫组织化学染色检测caspase-3与BDNF的表达,TUNEL法标记凋亡细胞;比较各组间差异。结果神经根压迫性损伤后,各时相点EPO给药组神经行为学特征恢复优于对照组(P<0.05);组织学染色显示在各时相点EPO给药组与NS对照组比较,受压神经根相对应的脊髓节段凋亡细胞数量减少(P<0.05),caspase-3蛋白的表达降低(P<0.05),BDNF蛋白的表达上调(P<0.05)。EPO硬膜外隙给药组与尾静脉给药组治疗效果差异无统计学意义。结论给予EPO能够减轻神经根因慢性压迫性损伤所致的神经功能减退及脊髓神经细胞的病理学改变,具有一定的神经保护作用。Objective To explore the protective effect of EPO on chronic compressive injury of lumbar nerve root in rat. Methods 54 SD rats were randomly divided into NS control group,EPO epidural space administration group and EPO caudal vein injection group. The animals in each group separately were fed for 1,2 and4 weeks. There were 6 rats at each group of different time point. During the surgery the rats' left L5intervertebral foramen was inserted a steel hollow tube to form a stable compression on the nerve root while the steel tube was socketed PE pipe and led to the neck subcutaneous to get epidural space administration. After membrane operation the NS control group and EPO epidural space administration group were respectively injected NS and EPO via the PE pipe in epidural space. The EPO caudal vein injection group was injected EPO via caudal tail vein. After the administration the rats' behavior changes were observed. He staining was used to examine the pathological changes of spinal cord nerve cell. Immunohistochemical staining was used to detect the expression of caspase- 3 and BDNF. Using TUNEL method to label apoptotic cells and comparing the differences between different groups. Results After compression injury on the nerve root in rats,neurological damage behavior of the rats were changed. The neurobehavioral characteristics of recovery in the group at each time point treated with EPO was better than the control group( P < 0. 05). The results of histological staining suggested that nerve cell in the spinal cord segment corresponding with nerve root compression from all of rats appears degeneration and apoptosis. At different time point,compared with NS control group,the number of apoptotic cells in the group treated with EPO was reduced( P < 0. 05),the amount of caspase- 3 protein expression was significantly decreased( P < 0. 05),the amount of BDNF protein expression was significantly increased( P < 0. 05).
