慢性阻塞性肺疾病急性加重期T波峰-末间期变化的临床研究

摘　　要：目的观察慢性阻塞性肺疾病急性加重期患者心电图T波峰-末间期(Tp-Te间期),并探讨其对慢性阻塞性肺疾病急性加重期患者近期预后的预测价值。方法测量95例慢性阻塞性肺疾病急性加重期患者和100例健康人心电图V2导联Tp-Te间期,并采用心率校正得出槡Tp-Te/RR。比较患者与健康组V2导联Tp-Te间期、槡Tp-Te/RR平均值的差别;根据患者住院期间死亡与否分为存活组和死亡组,比较两组V2导联Tp-Te间期、槡Tp-Te/RR平均值。结果慢性阻塞性肺疾病急性加重期患者V2导联Tp-Te间期、槡Tp-Te/RR平均值较健康对照组均明显延长,差异有统计学意义(P<0.05);在慢性阻塞性肺疾病急性加重期患者中,住院期间死亡组V2导联Tp-Te间期、槡Tp-Te/RR平均值较存活组均明显延长,且差异有统计学意义(P<0.05)。结论慢性阻塞性肺疾病急性加重期患者Tp-Te间期延长,Tp-Te间期变化与患者临床预后密切相关,对患者Tp-Te间期进行研究有助于患者临床预后的判定。Objective To observe the change of Tpeak-Tend(Tp-Te) intervals of patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD) and investigate its predictive value for recent prognosis. Methods The Tp-Te intervals of 95 AECOPD patients and 100 healthy cases were measured in V2 lead by 12-lead electrocardiogram(ECG) and were calculated Tp-Te /(1/2) RR corrected by heart rate. The differences of Tp-Te interval in V2 lead and the average value of Tp-Te /(1/2)RR were compared between patients and the healthy group. According to the clinical results,patients were divided into two groups,the survival group and the death group,among which the above two aspects were also compared. Results Compared with the healthy control group,AECOPD patients’ Tp-Te interval in V2 lead and the average value of Tp-Te / (1/2)RR were significantly prolonged and the differences were statistically significant(P &lt; 0. 05). Among AECOPD patients,Tp-Te interval in V2 lead and the average value of Tp-Te / (1/2)RR in the death group were obviously longer than those of the survival group,with statistically significant differences(P &lt; 0. 05). Conclusion For AECOPD patients,their Tp-Te intervals are prone to be prolonged. Since the change of Tp-Te intervals are closely related to clinical prognosis,it helps to judge the prognosis to probe into the subject.
