摘 要:目的探讨神经节苷脂对大鼠脊髓损伤(SCI)后神经元的保护作用及其机制。方法 66只Wistar雌性大鼠(体重2603^00 g)随机选取6只作为空白对照组(C组),6只采用改良Allen’s打击法于T<sub>9-11</sub>节段椎管制作大鼠急性SCI模型,随机分为实验组(A组)和生i理盐水对照组(B组),每组各30只。分别于术后1、3、7、14、21、28 d采用Rivilin斜板试验、改良Tarlov评分评价大鼠后肢运动功能恢复程度后处死收材,每组各时相点5只,以HE和免疫荧光染色观察大鼠SCI的修复情况。结果术后7 d,Rivilin斜板试验和Tarlov评分A、B组间比较,差异有统计学意义(P<0.05),A组功能恢复明显优于B组。术后28 d:HE染色示A组大鼠SCI处无明显空洞和瘢痕组织,有各种形态细胞形成,部分细胞有明显分化特征;B组SCI处被瘢痕组织填塞,可见大量炎性细胞和成纤维细胞浸润,并有较大脊髓空洞形成。免疫组织化学染色发现A组各时相点微管相关蛋白质-2(MAP-2)及胆碱乙酰转移酶(ChAT)表达的阳性细胞数较B组高,差异有统计学意义(P<0.05)。结论 SCI后大鼠神经功能的恢复与MAP-2及ChAT的表达呈一定相关性;单唾液酸四己糖神经节苷脂可通过增强SCI后MAP-2及ChAT的表达保护大鼠受损后脊髓的神经元。Objective To investigate the protective function and mechanism of ganglioside in neuronal cells after spinal cord injury(SCI).Methods Among 66 adult female Wistar rats(weighing260-300 g),6 of whom were randomly taken as the group of normal control,and others(60) were used to make acute SCI models at T_(9-11) level according to the improved Allen's method and were randomly divided into 2 groups(group A and B,n=30),the group of ganglioside(group A),and normal saline control(group B).At 1,3,7,14,21 and 28 days after operation,the nerve function of low extremities was evaluated using the Tarlov scale and the Rivilin inclined plane test,then executed the animals and got the needed tissue;the reparative effect of SCI was observed using immunohistochemical staining using 5 animals every time from each group.Results The Tarlov scale and the inclined plane test showed that the nerve function of low extremities in group A had got a better recovery from SCI than in group B after 7 days postoperation.Group A was significantly recovered compared with group B(P <0.05).28 days after operation.HE staining demonstrated that the injured region in group A restored well without obvious scar tissue and cavity,and there existed clear stem cell differentiation characters.There was great deal of scar tissue filled in the injury site in group B,where there were numerous inflammatory cells and fibroblasts infiltration and bigger cavity.Immunohistochemical staining showed that the expression of microtubule-associated protein 2(MAP-2) and choline acetyltransterase(ChAT) in group A was higher than in group B(P <0.05).Conclusion The recovery of nerve function of low extremities after SCI has some relationship with the expression of MAP-2 and ChAT and ganglioside can protect the neurons after SCI by promoting the expression of MAP-2 and ChAT.
