桡骨远端骨折合并下尺桡关节损伤非手术治疗的临床疗效

摘　　要：目的观察合并下尺桡关节损伤的桡骨远端骨折闭合复位石膏固定后的临床疗效。方法 46例合并下尺桡关节损伤的桡骨远端骨折患者进行闭合复位石膏同定,如1~3周复查见仍有尺骨背侧脱位情况,则改用前臂旋后位石膏固定。结果治疗4~6周,随访9~13月,平均11.3月,所有患者骨折均愈合。Gartland和Werley评分优40例,良3例,差3例,优良率93.4%。评分为良的3例患者中,腕关节桡偏畸形1例,另2例患者偶有疼痛,但无明显功能障碍。评分为差的3例患者均遗留有陈旧性下尺桡关节脱位。结论闭合复位石膏固定合并下尺桡关节损伤的桡骨远端骨折疗效确切,但需注意术后早期下尺桡关节复位丢失,如遗留陈旧下尺桡关节脱位,预后较差。对于关节面粉碎骨折伴有下尺桡关节损伤的病例较容易遗留陈旧性下尺桡关节脱位。Objective To study clinical outcome of distal radius fracture associated with distal radioulnar joint(DRUJ) injury.Methods 46 cases of distal radius fracture associated with DRUJ injury treated with closed reduction cast fixation were evaluated.Results Fractures were healed in all cases,and all patients were followed up in duration of 9-13 months(mean 11.3 months).Wrist function was evaluated with Gartland and Werley score system.The results of the recovery were 46 excellent,3 good and 3 bad,showing that 93.4%of patients restored excellent function of wrist and forearm.Conclusion Closed reduction cast fixation is fit for acute DRUJ.Bad outcome was associated with recurrent DRUJ luxation.
