急性胸痛伴心电图正常患者的冠状动脉造影及临床特点

摘　　要：目的探讨静息心电图正常的急性胸痛患者冠状动脉(冠脉)造影及临床特点。方法选择196例心电图无明显ST-T改变的患者作为研究对象,统计患者一般资料以及实验室相关辅助检查结果,所有入选患者最终均行诊断性冠脉造影术检查,以冠状动脉主要血管狭窄大于70%定为冠脉造影阳性,比较冠脉造影阳性与阴性患者的临床资料特点。结果 196例中有108例冠脉造影阳性,88例冠脉造影阴性。108例冠脉造影阳性患者中,冠脉多支血管病变82.4%;男性占75%;平均65.2岁;合并糖尿病、肾功能不全、高脂血症病史分别为54.6%、18.5%和87.0%,以上数据同冠脉造影阴性患者相比存在显著性差异(P<0.05)。结论静息心电图正常的胸痛患者冠脉病变发生与性别、年龄、合并其他高危因素有关,其中冠脉造影阳性患者多为冠状动脉多支血管病变。Objective To investigate the results of coronary angiography and clinical features in patients with acute chest pain and normal resting electrocardiogram .Methods 196 patients with acute chest pain but no significant ST-T changes were selected ,general information and laboratory examinations results are collected ,all pa-tients underwent diagnostic coronary angiography examination , the main coronary artery stenosis greater than 70%was defined as positive coronary angiography ,compared the clinical features and the results of coronary angiography between the positive and negative groups .Results Among the 196 patients,108 patients have one or more ramifi-cation lesions.Among these 108 patients, patients with multivessel lesions of coronary angiography account for 82.4%;male patients accounted for 75%;mean age was 65.2 years;the ratio of diabetes ,renal insufficiency and hyperlipidemia rates were 54.6%, 18.5% and 87.0% respectively , which were significant higher than those in control group(P<0.05 ).Conclusions In the patients with coronary artery disease with normal resting electrocar-diogram when chest pain occurs ,it came with different sex and age attribution ,and combined with other risk factors such as diabetes ,renal insufficiency and hyperlipidemia;besides,most of the patients were multivessel disease .
