经尿道膀胱腔内神经阻滞注射治疗女性间质性膀胱炎的疗效分析

摘　　要：目的:分析探讨采用经尿道膀胱腔内神经阻滞注射方式治疗间质性膀胱炎的有效性和可行性.方法:分析回顾2010年7月～2011年11月收治的女性间质性膀胱炎患者16例临床资料,临床表现为尿急、尿频或盆腔、会阴部的疼痛,术前均行膀胱镜检,病理证实为间质性膀胱炎,采用经尿道膀胱腔内神经阻滞注射方式进行治疗,对比分析患者手术前后疼痛指数评分和间质性膀胱炎症状和困扰指数评分的变化情况.结果:手术后2周,所有患者24 h排尿次数均明显下降,夜尿次数减少,膀胱容量较术前明显增加,患者疼痛指数、间质性膀胱炎症状指数及间质性膀胱炎困扰指数均较术前明显降低,均差异有统计学意义(P＜0.01).术后患者未见大量血尿、感染等并发症.结论:经尿道膀胱腔内神经阻滞注射可能是治疗女性间质性膀胱炎的一种有效的,可行的新方法,但其长期安全性及有效性有待于进一步证实.Objective:To investigate the feasibility and efficacy of treatment for 16 interstitial cystitis cases with transurethral intravesical nerve block injection.Methods:Between July 2010 and November 2011,16 interstitial cystitis cases who presented with urinary urgency and frequency or pelvic and perineal pain were analysed,the pathology was confirmed as interstitial cystitis with cystoscopy preoperatively,all cases had strong indication of surgery,using transurethral intravesical nerve block injection treatment with trigone injection of anesthetic and glucocorticoid drugs and comparative analysing the patients pain index score and interstitial cystitis symptoms and distress index score changes before and after surgery.Results:Two weeks after surgery,all patients' a 24-hour voiding and nocturia frequency decreased significantly,bladder capacity increased significantly compared with the preoperative,the pain scores and cystitis symptom index,interstitial cystitis plagued index decreased significantly compared with the preoperative,the differences were statistically significant(P<0.01).The postoperative patients were not seen a large number of hematuria,infection or other complications.Conclusions:Transurethral intravesical nerve block injection may be a new method treating for interstitial cystitis,which is effective and feasible,but its long-term safety and efficacy remains to be further confirmed.
