后腹腔镜手术治疗肾上腺囊肿22例

摘　　要：目的:探讨后腹腔镜手术治疗肾上腺囊肿的安全性及效果.方法:2002年5月~2011年7月,共采用后腹腔镜手术治疗肾上腺囊肿22例,囊肿直径2.0~12.7 cm,平均5.9 cm.结果:所有手术均成功完成,其中21例为后腹腔镜一侧肾上腺囊肿切除术,1例为后腹腔镜双侧肾上腺囊肿切除术.手术时间50~190 min,平均99 min;出血10~200 ml,平均32 ml;术后住院3~32 d,平均6.8 d;术后1例出现小腿肌间静脉血栓,予对症治疗后好转.术后留置肾周引流管3~9 d,平均4.5 d;术后随访8~118个月,平均40.5个月,未发现复发.结论:后腹腔镜手术治疗肾上腺囊肿创伤小、安全、效果满意,而且易于掌握,是治疗肾上腺囊肿的有效方法.Objective:To evaluate the safety and efficacy of retroperitoneal laparoscopic surgery in the treatment of adrenal cysts.Methods:A total of 22 patients with adrenal cysts were treated with retroperitoneal laparoscopic op- eration from May 2002 to July 2011.The diameters of cysts ranged from 2.0cm to 12.7cm,and the mean diameter was 5.9cm.Results:The procedure was completed successfully in all cases.21patients were diagnosed with a unilat- eral cyst,and 1patient with bilateral ones.Mean operative time was 99min(range,50-190min)and mean blood loss was 32ml(range,10-200ml).The mean postoperative hospital stay was 6.8days(range,3-32days).During hospitalization,venous thrombosis of the lower extremity occurred in one patient who was cured later.Perirenal drainage tubes were placed for 3-9days and the mean time was 4.5days.Patients were followed up for 8-118months(mean,45months)without cyst recurrence.Conclusions:Retroperitoneal laparoscopic surgery for adrenal cysts is a safe,effective,less invasive method for adrenal cysts and can be easy to learn.
