双重定位法在PCNL治疗鹿角形肾结石术中的应用(附136例报告)

摘　　要：目的:探讨双重定位法(联合B超、X线进行定位)在经皮肾镜取石术(PCNL)治疗鹿角形肾结石中的应用价值.方法:回顾性分析2008年12月~2013年5月间采用双重定位法的PCNL术治疗136例鹿角形肾结石的临床资料.其中完全鹿角形结石79例,不完全鹿角形结石57例.结果:136例均手术成功.其中一期手术89例次,二期手术45例次,三期手术14例次,单通道72例次,双/多通道76例次.平均手术时间79.8 min.结石清除率91.2%(135/148).无严重并发症病例.结论:应用双重定位法的PCNL术在治疗鹿角形肾结石方面具有有效、安全的特点,能提高清石率,降低并发症发生概率,在有条件实施的情况下值得推荐.Objective:To evaluate the therapeutic effect of percutaneous nephrolithotomy(PCNL)under dual guidance(ultrasonic and X-ray guidance)in the treatment of staghorn calculi.Methods:A retrospective analysis was conducted among 136 patients undergoing PCNL under dual guidance for staghorn calculi(89 cases with complete staghorn calculi and 59cases with partial staghorn calculi)from Dec.2008to May.2013.Results:All operations were successful without severe complications,of which a single tunnels were used in 72 cases,2-3 tunnels were used in 76 cases.89cases had a single session PCNL,45 cases had a second-lock PCNL,and 14 cases had a third-lock PCNL. The average operating time was 79.8 min.The stone free rate reached 91.2%(135/148).Conclusions:PCNL under dual guidance is safe and effective in treatment of staghorn calculi and is associated with high stone free rate and few postoperative complications.It is recommended in the hospital on conditional implementation.
