妊娠中晚期高脂血症性胰腺炎14例临床诊治分析

摘　　要：目的探讨妊娠中晚期伴发高脂血症性胰腺炎的临床特点和治疗经验。方法 14例妊娠中晚期合并高脂血症性胰腺炎的患者在密切观察病情的前提下,根据孕周分别予以药物保守治疗,或及时采取手术终止妊娠、清除胰腺坏死组织,术后应用血液净化联合血浆置换治疗。结果 1例母婴死亡,13例母亲痊愈出院,保胎成功9例。结论妊娠中晚期合并高脂血症性胰腺炎的早期诊断非常重要,根据孕周及病情变化合理选择个体化治疗方案是治疗成功的关键。Objective To discuss the clinical characteristics,the diagnosis and treatment of the patients with acute hyperlipidemic pancreatitis in middle and late pregnancy stage.Methods A retrospective analysis including the diagnosis,therapeutic modality,effect and postoperative complications was made for 14 patients with acute hyperlipidemic pancreatitis.Results Of all patients, 13 patients were discharged from hospital after their entire recovery except 1 patients fetus died and 9 patients fetuses were successfully saved . Conclusion Since clinical presentation of acute hyperlipidemia pancreatitis is complex,the misdiagnosis is more common and this disease seriously threatens the life of mother with their fetus.It is very important to make an early correct diagnosis and proper individualized therapy principles for the patients with acute hyperlipidemic pancreatitis during the midterm and late stage of pregnancy.
