代谢综合征患者心率变异性影响因素分析

摘　　要：目的探讨代谢综合征患者心率变异性变化特点。方法选择1999年7月~2005年6月在我院糖尿病中心确诊的代谢综合征189例作为病例组,同期本院工作人员及门诊体检健康者162例作为对照组,比较两组心率变异性各项指标;同时对病例组糖尿病病史、血压、甘油三酯、高密度脂蛋白胆固醇和腰臀围比,按其病程长短或是否正常分别进行分组比较,再进行多因素回归分析。结果代谢综合征患者与对照组比较,心率变异性各项指标有明显降低(F=20.123~150.240,P【0.01)。糖尿病病程长、高血压、腰臀围比异常患者心率变异性的部分频域或时域指标降低(t=-0.262~0.268,P【0.05)。结论代谢综合征患者自主神经功能均有损害,糖尿病病程、高血压和腰臀围比异常是其重要的影响因素。Objective AIM To study the characteristics of heart rate variability(HRV)in patients with metabolism syndrome(MS). Methods Total 189cases of MS were enrolled during July 1999to June 2005in the Diabetes Center of this hospital,and 162healthy controls were selected during the same period.The parameters of heart rate variability were compared between the two groups.Meanwhile, history of diabetes,blood pressure,blood lipids,high density lipoprotein cholesterol and waist-hip ratio by length in MS patients were measured and divided into different group,based on abnormal or normal results of the above parameters.The multivariate regression analysis was performed between two groups. Results Compared with the control group,HRV was significantly reduced in patients with MS(F= 20.123~150.240,P<0.01).In MS patients with history of diabetes,high blood pressure,and abnormal waist-hip ratio,partial frequency domain or time domain index of the heart rate variability were decreased compared with control group(t=-0.262~0.268,P<0.05). Conclusion Autonomic function in MS patients has varying degrees of damage,and a history of diabetes,high blood pressure,and waist-hip ratio affect the autonomic function in the patients.
