α1A-受体拮抗剂坦索罗辛作为潜在性男性避孕药的临床研究

摘　　要：目的探讨坦索罗辛作为一种潜在的男性避孕药应用的有效性和安全性。方法 40名受试者随机分为2组(A组和B组),按依次交叉的方式给予安慰剂和坦索罗辛(单次口服剂量为0.4 mg或0.8 mg),每次给药之间有7天的间歇期。每次给药后4~6小时,评估每个受试者的射精情况,同时记录性欲和坦索罗辛的不良反应。结果服用0.8 mg坦索罗辛后,所有受试者均发生了不射精症;服用0.4 mg的坦索罗辛后,有功能的精子总数显著减少。6名受试者表示服用0.8 mg的坦索罗辛后出现可忍受的不适,其中1名出现眼睑下垂、2名出现鼻塞、3名出现乏力。症状在服药10小时后消失。结论坦索罗辛可导致男性射精异常,且存在剂量相关性。α1A-肾上腺素受体拮抗剂坦索罗辛在将来可能成为一种男性避孕药,需进一步研究证实。Objective To investigated the effectiveness and safety of tamsulosin as a potential male contraceptive.Methods Forty healthy male volunteers were enrolled.Conditions that mimic the adverse effects of α1-adrenoceptor antagonists such as headache,dizziness,rhinitis,diarrhea,thirst,hypotension,etc.,were examined.This study was in a placebo controlled,randomized and crossover design.We administered placebo and tamsulosin(at a single oral dose of 0.4-mg or 0.8-mg) sequentially in a crossover manner in order to eliminate bias among drugs and doses.The volunteers were equally divided into two groups(A and B).Ejaculatory profiles were assessed by masturbation 4-6 h after each administration,including conventional semen analysis and sperm count in midstream urine after ejaculation(in case of anejaculation).Libido and adverse effects of tamsulosin were noted.Seven days after the last administration,endpoint ejaculatory profiles were evaluated in all volunteers by masturbation.Results Anejaculation occurred in all subjects after taking 0.8 mg of tamsulosin.Total functional sperm count was significantly reduced in subjects after taking 0.4 mg of tamsulosin.Six subjects receiving 0.8 mg of tamsulosin complained of tolerated discomfort which disappeared 10 hours after administration.Conclusions When administered at 0.8 mg,tamsulosin can cause anejaculation with some transient side effects.Our results implie that tamsulosin and related drugs might potentially be used as male contraceptive agents in the future,which needs more studies to verify.
