基质金属蛋白酶1,9表达与下肢慢性静脉功能不全关系的研究

摘　　要：目的检测慢性静脉功能不全(CVI)患者大隐静脉组织中基质金属蛋白酶(MMP)1,9的表达,探讨MMP-1、MMP-9在CVI血管重塑中的作用。方法对80例CVI患者(其中轻度CVI22例,中度CVI28例,重度CVI30例)及20例对照组病例,采用免疫组织化学染色法测定MMP-1、MMP-9蛋白在大隐静脉标本中的表达,以逆转录-聚合酶链反应(RT-PCR)法检测MMP-1mRNA、MMP-9mRNA的表达。结果在轻、中、重CVI组中MMP-1蛋白及mRNA的表达水平高于正常对照组,差异有统计学意义(P<0.05);在轻、中、重CVI组中MMP-9蛋白及mRNA表达水平高于正常对照组,差异有统计学意义(P<0.05);在轻、中、重CVI三组内比较时,重度CVI组和轻、中度CVI组之间MMP-9mRNA表达,差异有统计学意义(P<0.01)。结论 MMP-1、MMP-9可能参与下肢CVI血管重塑这个过程的调节;MMP-9表达水平随临床严重程度升高而升高,提示MMP-9可能作为评估临床严重程度的一个指标。Objective To examine the expression of matrix metalloproteinase-1(MMP-1) and matrix metalloproteinases-9(MMP-9) in patients with chronic venous insufficiency(CVI),and to evaluate the role of MMP-1 and MMP-9 during vascular remodeling in CVI. Methods Eighty patients with CVI were classified into mild(n=22)、moderate(n=28) and severe(n=30) groups; 20 cases without CVI were allocated to control group. MMP-1、MMP-9 expression in the great saphenous vein were tested with immunohistochemical method. Expression of MMP-1 mRNA and MMP-9 mRNA were measured with the Reverse transcription-polymerase chain reaction(RT-PCR). Results Expressions of MMP-1 protein, MMP-9 protein,MMP-1 mRNA and MMP-9 mRNA in mild, moderate and severe group were significantly higher than those in control group (P<0.05). Between different CVI groups, the expressions of MMP-9 mRNA in severe group were significantly higher comparing to those in mild and moderate group (P<0.01). Conclusion MMP-1 and MMP-9 may be involved in the regulation of CVI vascular remodeling. Level of MMP-9 was increased exponentially with the clinical severity of CVI, therefore it may be an indicator when assessing severity of CVI.
