周围型小肺癌“颗粒征”影像动态观察

摘  要:目的:探讨小肺癌灶(直径<3.0 cm)“颗粒征”影像的动态表现,以期提高对小肺癌的诊断率。方法收集经手术、病理、细胞学及临床证实的45例小肺癌患者,选取具有小斑点、斑片或结节状征象者30例作为观察对象,分析其动态变化。全部病例均摄有常规 X胸片(2次以上)并作同期肺部 CT扫描。结果30例患者 X线片、肺部CT扫描的影像学表现均有从小到大,密度从淡变浓,由形态多样到融合成团块,从不典型到典型的分叶、毛刺、棘状突起或血管改变等综合影像。2月以上复查成结节肿块者8例,1年以后有变化者12例,3~4年有变化者6例,5~6年以上者4例。总计恶性结节22例,良性结节8例。结论“颗粒征”在周围型肺癌的诊断中有重要价值,为小肺癌早期手术提高生存率有重要意义。Objective To study the dynamic performance for “particles character”imaging of small lung cancer lesions (diameter<3.0 cm)in order to improve diagnosing small lung cancer.Methods 45 cases with small lung cancer proved by operation,pathological,cytological and clinical verification in our hospital were selected,while 30 cases with dots,patches or nodular signs were chosen for observation ob-jects for analyzing their dynamic changes.All patients were taken conventional chest X (>2 times)and concurrent pulmonary CT scanning.Results Both X-ray film and CT scanning of 30 cases showed inte-grated image from small to large,density from weak to thick,morphology from diversity to integrate into clumps,non-typical to typical leaf,burr,spines,or vascular changes and etc.For obser Vation of 12 cases which changed into nodular masses after being reviewed for more than 2 months,8 cases had chan-ges after 1 year, 6 cases had changes 3~4 years later, and 4 cases had changes after more than 5~6 years.Among total of 22 cases of malignant nodules,there are 8 cases with benign nodules.Conclusion“Particles character”has an important value for the diagnosis of peripheral lung cancer,and has important significance to the early operation for increasing the survival rate.
