子宫内膜癌术后10年生存因素的分析

摘　　要：目的：探讨影响子宫内膜癌根治术后的生存因素。方法对126例子宫内膜癌Ⅰb～Ⅱa期根治术后生存10年患者的临床资料进行回顾性分析。应用Kaplan-Meier法计算累计生存率，各因素间比较采用卡方检验。并对各临床参数进行单因素和COX模型的多因素分析。结果3年以上生存率达92％（155/169），5年以上生存率达82％（139/169）、10年以上生存率达74％（126/169）。分析结果表明：影响生存率的主要因素是细胞学分级、肌层浸润、淋巴结转移、术后是否行辅助治疗（P＜0．05）；而肿瘤大体分型对生存率无显著影响（P＞0．05），术后放疗、化疗及孕激素治疗对患者生存率的影响无显著性差异（ P＞0．05）。结论早期发现、早期诊断是提高子宫内膜癌患者术后10年生存率的前提，早期手术治疗及术后辅助治疗是提高患者术后10年生存率的关键。Objective To study the survival factors of endometrial cancer after radical resection .Methods Retrospec-tive analysis of clinical data of 126 cases ofⅠb-Ⅱa endometrial cancer who underwent radical resection in 10 years.The cumula-tive survival rate was calculated by Kaplan-Meier method,the factors were compared with the chi-square test.Multiple factors a-nalysis and single factor and COX model in various clinical parameters .Results The 3 year survival rate was 92%(155/169), the 5 year survival rate was 82%(139/169),the 10 year survival rate was 74%(126/169).The results showed that the main factors affecting the survival rate were cytologic grading ,myometrial invasion,lymph node metastasis,postoperative therapy (P<0.05);and the tumor size had no significant effect on survival rate (P>0.05),Effect of postoperative radiotherapy ,chemothera-py and hormone on the survival rate had no significant difference (P>0.05).Conclusion Early detection,early diagnosis are the precondition to improve the survival rate of patients of endometrial cancer in 10 years,early operation and postoperative adju-vant therapy are the key to improve the postoperative 10-year survival rate .
